摘  要:目的:探究体外冲击波联合盐酸氨基葡萄糖片治疗颞下颌关节紊乱综合征的效果。方法:选取2019年1月~2020年1月我院收治并确诊的颞下颌关节紊乱综合征患者80例为研究对象,按治疗方式的不同分为联合组和对照组各40例,对照组使用盐酸氨基葡萄糖片治疗,联合组在对照组基础上采用体外冲击破治疗。对照组治疗4~12周,联合组治疗5~7 d治疗1次,4次为1个疗程。检测所有患者治疗前后的最大张口度、临床功能障碍指数评分、视觉模拟评分法(visual analogue scale,VAS)、口腔健康影响程度量表(oral health impact profile-14,OHIP-14)、Fricton指数法评价下颌功能,以及关节液中相关因子水平并统计不良反应。结果:与对照组相比,联合组治疗后Heikimo、OHIP-14、JN、MM、MP、JP评分以及TNF-α、IL-1水平均显著降低,最大张口度、DI评分以及TGF-β1、IGF-1水平均显著升高(P<0.05)。与对照组相比,联合组治疗总有效率较高(P<0.05)。结论:盐酸氨基葡萄糖片基础上给予体外冲击波可用于治疗颞下颌关节紊乱综合征,有效缓解骨关节的疼痛症状,改善关节功能,抑制炎症因子表达,缓解关节软骨损伤。Objective:To explore the effect of extracorporeal shock wave combined with glucosamine hydrochloride in the treatment of temporomandibular disorder syndromes.Methods:From January 2019 to January 2020,80 patients with temporomandibular disorder syndromes who were admitted and diagnosed in our hospital were selected into our study.According to the different treatment methods,they were divided into the combination group and the control group,40 cases in each group.The control group was given glucosamine hydrochloride tablets,the combination group was treated with extracorporeal shock therapy based on the control group.The control group was treated for 4~12 weeks,and the combined group was treated for 5~7 days per time,and 4 times as a course of treatment.The maximum mouth opening,clinical dysfunction index score,visual analogue scale(VAS),oral health impact profile-14(OHIP-14),Fricton index and interleukin-1(IL-1),tumor necrosis factor-α(TNF-α),insulin-like growth factor-1(IGF-1),transforming growth factor-β1(TGF-β1)before and after treatment of 2 groups were collected and compared.The adverse reactions of 2 groups were also compared.Results:After treatment,compared with the control group,the Heikimo,OHIP-14 and JN,MM,MP,JP,TNF-α,IL-1 levels were significantly reduced,the maximum mouth opening,DI,TGF-β1,IGF-1 were significantly increased in the combination group(P<0.05).Compared with the control group,the total effective rate of the combination group was higher(P<0.05).Conclusion:Based on the glucosamine hydrochloride tablets,extracorporeal shock waves can be used to treat temporomandibular joint disorder syndrome by effectively alleviating the pain of bone and joint,improving joint function,inhibiting the expression of inflammatory factors,and relieving articular cartilage damage.
