摘 要:目的探讨虚拟现实技术对脑卒中后运动性失语患者语言功能的治疗作用。方法选取脑卒中后运动性失语患者18例,随机分为观察组与对照组。观察组给予常规语言训练、虚拟现实技术(virtual reality,VR)训练各20 min/天,两种训练间休息5 min;对照组给予常规语言训练40 min/天。两组患者训练5天/周,共4周。训练前后采用汉语标准失语症检查(Chinese rehabilitation research center standard aphasia examination,CRRCAE)进行语言能力评估;采用波士顿诊断性失语症检查(the boston diagnostic aphasia examination,BDAE)评定失语症严重程度。结果训练后,两组患者失语症严重程度均有显著改善(P<0.05),观察组改善程度显著优于对照组(P<0.05);患者听理解、复述、表达、出声读、阅读、抄写、描写、听写能力均显著提高(P<0.05);观察组患者名词复述、句子复述、名词表达、动词表达、句子表达、漫画表达、列举、动词出声读、动词阅读、句子阅读、名词抄写、句子抄写、动词描写等语言能力极显著优于对照组(P<0.01)。结论虚拟现实技术结合语言功能训练比单纯语言功能训练能够改善运动性失语患者的语言能力。Objective To explore the therapeutic effect of virtual reality technology on speech function of Broca Aphasia patients after stroke.Method 18 patients with stroke were enrolled in this study.They were randomly divided into two groups.The observation group received regular speech training for 20 minutes per day,Virtual Reality(VR)training for 20 minutes per day;the control group was given conventional speech training 40 minutes per day.And both groups were trained for 5 days per week lasting for 4 weeks.Chinese rehabilitation research center standard aphasia examination(CRRCAE)was used to assess their speech competence.The Boston Diagnostic Aphasia Examination(BDAE)was used to assess the severity of aphasia.Results After 4 weeks,the severity of aphasia was improved in both groups,while the degree improved of the observation group was signifi cantly better than the control group(P<0.05).Listening comprehension,retelling,expression,reading aloud,reading,transcription,description,and dictation of both groups were all improved(P<0.05).And the grade of noun repetition,sentence repetition,noun expression,verb expression,sentence expression,comic expression,enumeration,verb readout,verb reading,sentence reading,noun transcription,sentence transcription and verb depiction of the observation group was significantly better than the control group(P<0.01).Conclusion Virtual reality combined with speech function training can improve the speech competence of patients with Broca Aphasia,and its improvement is better than the traditional speech function training.
