腹腔镜下TME术联合ISR术治疗超低位直肠癌的效果分析

摘　　要：目的:分析超低位直肠癌应用腹腔镜全直肠系膜切除术(TME)联合经肛门内括约肌间切除术(ISR)的疗效。方法:笔者所在医院收治的超低位直肠癌患者60例,随机分为观察组和对照组,各30例,观察组行腹腔镜TME+ISR,对照组行开腹ISR,比较两组围术期指标、肛门功能、并发症及预后效果。结果:观察组临床指标优于对照组,保肛率高于对照组(93.33%vs 60.00%),肛门功能优于对照组,并发症率低于对照组(6.67%vs 26.67%),差异均有统计学意义(P<0.05);两组复发率及转移率等差异无统计学意义(P>0.05)。结论:腹腔镜下TME联合ISR治疗低位直肠癌,创伤小、胃肠功能恢复快,肛门功能高,并发症率低,患者生活质量高,值得临床推广。Objective:To analyze the efficacy of laparoscopic total mesenterectomy(TME)combined with transanal internal sphincterectomy(ISR)for ultralow rectal cancer.Method:A total of 60 ultra-low rectal cancer patients admitted to our hospital were randomly divided into observation group and control group with 30 cases each.The observation group underwent laparoscopic TME+ISR,and the control group underwent laparotomy ISR.The perioperative indicators and anus function,complications and prognostic effects were compared between the two groups.Result:The clinical indicators of the observation group were better than those of the control group,the anus preservation rate was higher than that of the control group(93.33%vs 60.00%),the anal function was higher than that of the control group,and the complication rate was lower than that of the control group(6.67%vs 26.67%),the differences were all significant(P<0.05),the recurrence rate and metastasis rate of the two groups were not statistically significant(P>0.05).Conclusion:The combination of TME and ISR under laparoscopic treatment for low rectal cancer has less trauma,quick recovery of gastrointestinal function,high anal function,low complication rate,and high quality of life for patients.It is worthy of clinical promotion.
