摘　　要：目的:基于临床各指标变化分析血液灌流联合低分子肝素治疗高脂血症性急性胰腺炎的应用价值。方法:选取2017年1月-2020年4月笔者所在医院收治的90例高脂血症急性胰腺炎患者,按随机数字表法将其均分为两组,对照组采用血液灌流治疗,观察组在对照组的基础上联合低分子肝素治疗。观察两组临床疗效、治疗前后TG水平及APACHE Ⅱ评分,对比两组治疗前后炎症因子水平。结果:观察组治疗总有效率(97.78%)高于对照组(84.44%),差异有统计学意义(P<0.05);治疗前两组TG水平、APACHE Ⅱ评分及各炎症因子水平比较差异均无统计学意义(P>0.05),治疗后观察组TG水平、APACHE Ⅱ评分、CRP、TNF-α及IL-6水平均低于对照组,差异均有统计学意义(P<0.05)。结论:对高脂血症性急性胰腺炎患者采取血液灌流及低分子肝素联合治疗效果明显,能减轻患者体内炎性反应,降低甘油三酯含量,同时降低APACHE Ⅱ评分,显著改善患者病情严重程度,增强预后效果,值得临床运用。Objective:To analyze the application value of hemoperfusion combined with Low Molecular Weight Heparin in treatment of hyperlipidemic acute pancreatitis based on the changes of clinical indicators.Method:A total of 90 patients with hyperlipidemia acute pancreatitis admitted to our hospital from January 2017 to April 2020 were selected and divided into two groups according to random number table method,the control group was treated with hemoperfusion and the observation group was treated with Low Molecular Weight Heparin on the basis of the control group.The clinical efficacy,the changes of TG level and APACHEⅡscore before and after treatment were observed,the changes of inflammatory factors before and after treatment were compared between the two groups.Result:The total effective rate(97.78%)in the observation group was higher than that in the control group(84.44%),the difference was statistically significant(P<0.05).Before treatment,there were no significant differences in TG level,APACHEⅡscore and inflammatory factors between the two groups(P>0.05),after treatment,TG level,APACHEⅡscore,CRP,TNF-αand IL-6 levels in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Hemoperfusion combined with Low Molecular Weight Heparin has a significant effect on patients with hyperlipidemia acute pancreatitis,which can reduce the inflammatory response in patients,reduce the content of triglyceride,and reduce APACHEⅡscore at the same time,significantly improve the severity of the patients’condition,enhance the prognosis,and is worthy of clinical application.
