腮腺淋巴上皮囊肿的影像学特征分析

摘　　要：目的:分析腮腺淋巴上皮囊肿的CT及MRI影像学表现特征。方法:回顾性分析笔者所在医院2012年1月-2018年1月经手术与病理证实的21例腮腺淋巴上皮囊肿患者的临床资料,分析其影像学特征并查阅相关内容文献复习。结果:此组21例病例中,误诊10例,分别误诊为多形性腺瘤4例、神经源性肿瘤2例、腮裂囊肿1例、淋巴结感染并囊变1例,病灶多位于腮腺浅叶,单侧单个病灶为主,双侧多个病灶少见,病灶多为椭圆形或类圆形,边界清晰。CT主要表现为腮腺内单发的薄壁类圆形或椭圆形囊性病灶,边界清晰,囊内呈低密度或稍高密度改变,增强扫描囊壁呈轻至中度强化,囊内均无强化。MRI主要表现为腮腺内单发的薄壁囊性病灶,类圆形或椭圆形,边界清晰,囊壁呈低信号,囊内T_1WI呈低信号,T_2WI呈高信号,增强扫描囊壁均呈明显强化,囊内均无强化。结论:腮腺淋巴结上皮囊肿多为单发薄壁囊性病变,其CT及MRI的影像表现具有典型的特征。Objective:To analyze the CT and MRI imaging features of parotid lymphoepithelial cyst.Method:The clinical data of 21 patients with parotid gland lymphoepithelial cyst confirmed by menstrual surgery and pathology in our hospital from January 2012 to January 2018 were retrospectively analyzed.The imaging features were analyzed and relevant literature was reviewed.Result:Among the 21 cases in this group,10 cases were misdiagnosed as pleomorphic adenoma(4 cases),neurogenic tumor(2 cases),parochial cleft cyst(1 case),lymph node infection and cystic degeneration(1 case).Most of the lesions were located in the superficial lobe of parotid gland,with unilateral single lesion as the dominant one.Bilateral multiple lesions were rare,and most of the lesions were elliptic or round,with clear boundaries.CT mainly showed single thin-walled round or elliptic cystic lesions in the parotid gland,with clear boundaries,low-density or slightly high-density changes in the cyst,enhanced scan showed mild to moderate enhancement in the cyst wall,and no enhancement in the cyst.MRI mainly showed single thin-walled cystic lesions in the parotid gland,like round or oval,with clear boundaries,low signal in the cyst wall,low signal in T1WI and high signal in T2WI,and obvious enhancement in the cyst wall,but no enhancement in the cyst.Conclusion:Most of the cysts in the lymph nodes of parotid gland are single thin-walled cystic lesions,and the CT and MRI images have typical characteristics.
