参芪归脾汤加味内服和保留灌肠对结直肠癌化疗患者的影响

摘　　要：目的:探讨参芪归脾汤加味内服与保留灌肠对结直肠癌化疗患者相关毒副反应的预防作用及其对患者细胞免疫功能的影响。方法:将结直肠癌患者130例采用随机数字表法分为观察组和对照组各65例,观察组患者术后采用参芪归脾汤内服和保留灌肠结合mFOLFOX6化疗,对照组术后采用mFOLFOX6化疗,观察并记录两组患者化疗过程中的毒副反应发生情况,测试患者化疗前后细胞免疫功能,评价两种方案的可行性与安全性。结果:IgA、IgM及IgG两组治疗后均较治疗前下降(P<0.05),观察组降低幅度低于对照组;治疗后观察组外周血中辅助T细胞、细胞毒性T细胞、B细胞和NK细胞表达均高于对照组(P<0.05);观察组各项毒副反应发生率均低于对照组(P<0.05),同时观察组患者生存质量更高(P<0.05)。结论:参芪归脾汤内服和保留灌肠联合常规化疗对结直肠癌化疗患者的毒副反应抑制作用明显,且相比单纯化疗对患者细胞免疫功能的影响更小,有利于提高患者预后生存质量。Objective:To investigate the preventive effects of oral administration and retention enema of modified Shenqi Guipi decoction on the chemotherapy-related toxic reactions in the patients with colorectal cancer after chemotherapy,and its influence on cellular immune function.Methods:All 130 patients were divided into the observation group and the control group according to random number table method,the observation group accepted oral administration and retention enema of modified Shenqi Guipi decoction combined with mFOLFOX6 chemotherapy after operation,and the control group only used mFOLFOX6 chemotherapy after operation,to observe and record the incidence of chemotherapy-related toxic reactions during the course,detect ceullular immune function before and after chemotherapy,and assess the feasibility and safety of the two schemes.Results:The levels of IgA,IgM and IgG reduced after treating than before treating in both groups(P<0.05),the decrease of the observation group was lower than that of the control group;after the treatment,the expressions of T helper cells,cytotoxic T cells,B cells,NK cells of the observation group were higher than these in the control group(P<0.05);the observation group was lower than the control group in the incidences of toxic reactions(P<0.05),meanwhile,quality of life in the observation group was higher(P<0.05).Conclusion:Oral administration and retention enema of modified Shenqi Guipi decoction combined with conventional chemotherapy show notable inhibitory effects on toxic reaction in the patients,it possesses less impacts on cellular immune function in patients compared with single chemotherapy,and it is helpful to raise quality of life in the patients.
