磁共振波谱联合视频脑电图对伴海马硬化的内侧颞叶癫痫的诊断价值

摘　　要：目的探讨磁共振波谱(MRS)联合视频脑电图(V-EEG)诊断伴有海马硬化的内侧颞叶癫痫(MTLE-HS)的价值。方法选择2017年7月至2019年7月于我院确诊的55例MTLE-HS患者作为研究对象,将其划分为耐药组和有效组,根据V-EEG监测结果再将其划分为单侧放电组、双侧放电组;选取同期接收并体检健康者21名作为健康对照组。所有入选者均行MRI扫描、双侧海马体MRS诊断,比较各组海马体各指标比值、单双侧异常放电一致性等。结果MTLE-HS患者行头颅MRI诊断后,得出颞叶异常改变者共32例,检出率为58.18%;耐药组、有效组的V-EEG、MRS检出单侧异常放电一致性分别为85.71%、75.86%,双侧异常放电一致性分别为100%、42.86%;耐药组的NAA/(Cho+Cr)、NAA/Cho、NAA/Cr的比值均低于有效组,但仅NAA/Cr的比值有统计学的意义(P<0.05)。结论MRS联合V-EEG诊断MTLE-HS的价值较高,能够为临床诊治提供重要的参考依据。Obstract To study the value of magnetic resonance spectroscopy(MRS)combined with video electroencephalogram(V-EEG)in the diagnosis of medial temporal lobe epilepsy(MTLE-HS)with hippo-sclerosis.Methods Select 55 MTLE-HS patients diagnosed in our hospital from July 2017 to July 2019 as the research object,divide them into drug-resistant group and effective group,and divide them into unilateral discharge based on the V-EEG monitoring results group,bilateral discharge group.Twenty-one healthy peoples were selected as the control group.MRI scan and bilateral MRS diagnosis were performed in all groups,and the ratios of various indicators of the hippocampus and the consistency of unilateral and bilateral abnormal discharge were compared.Results After cranial MRI diagnosis of patients with MTLE-HS,32 patients were found to have abnormal temporal lobe changes,with the detection rate of 58.18%.The consistency of unilateral abnormal discharge detected by V-EEG and MRS in the drug-resistant group and the effective group was 85.71%and 75.86%,respectively,and the consistency of bilateral abnormal discharge was 100%and 42.86%,respectively.Although the NAA/(Cho+Cr),NAA/Cho and NAA/Cr ratios in the drug-resistant group were lower than those in the effective group,only the NAA/Cr ratios were statistically significant(P<0.05).Conclusion The value of MRS combined with V-EEG in the diagnosis of medial temporal lobe epilepsy is higher,which can provide an important reference for clinical diagnosis and treatment.
