EEN联合阶梯式引流治疗重症胰腺炎坏死感染的疗效观察

摘　　要：目的分析早期肠内营养(EEN)联合阶梯式引流治疗重症胰腺炎坏死感染的可行性、效果。方法选择我院84例重症胰腺炎坏死感染患者,予以患者禁食、禁饮等基础治疗基础上经伦理委员会批准,进行对照组、研究组分组(各42例)治疗。对照组患者采取阶梯式引流治疗,研究组患者采取EEN联合阶梯式引流治疗。结果组间临床疗效情况比较,研究组患者总有效率95.23%高于对照组(P<0.05);治疗后,两组患者的炎性因子、血清营养学指标水平比较治疗前改善,并且研究组优于对照组(P<0.05);组间并发症发生情况比较,研究组腹腔感染等并发症发生率2.38%低于对照组(P<0.05)。结论禁食、禁饮等基础治疗辅助EEN联合阶梯式引流治疗重症胰腺炎坏死感染可以有效纠正炎性水平、改善机体营养状态,提升了治疗效果与安全性。Objective To analyze the feasibility and effect of early enteral nutrition(EEN)plus stepped drainage in the treatment of necrotic infection of severe pancreatitis.Methods Eighty-four patients with severe pancreatitis necrotizing infection in our hospital were treated with fasting,drinking and other basic treatment,and were divided into control group and study group(42 cases in each group)with the approval of ethics committee.The control group was treated with stepped drainage,and the study group was treated with EEN combined with stepped drainage.Results The total effective rate of the study group was 95.23%higher than that of the control group(P<0.05);the levels of inflammatory factors and serum nutritional indexes were improved after treatment,and the study group was better than the control group(P<0.05);the incidence of complications in the study group was 2.38%lower than that in the control group(P<0.05).Conclusion Fasting,drinking and other basic treatment combined with EEN combined with stepped drainage in the treatment of severe pancreatitis necrotic infection can effectively correct the level of inflammation,improve the nutritional status of the body,and improve the therapeutic effect and safety.
