局部进展期直肠癌PARP-1表达与临床转归的关系

摘　　要：目的探讨局部进展期直肠癌聚腺苷二磷酸核糖聚合酶-1(PARP-1)表达与临床转归的关系。方法选取2008年1月至2012年11月浙江大学医学院附属邵逸夫医院收治的中低位局部进展期直肠癌患者50例,所有患者接受术前新辅助放化疗+全直肠系膜切除术+术后辅助化疗。取患者治疗前肠镜活检肿瘤组织标本,采用免疫组化SP染色法检测PARP-1表达,比较PARP-1高、低表达患者术后复发转移及生存情况。结果所有患者随访47.85个月,发生转移复发13例,转移复发率为26.0%;中位无病生存期为46.75个月,1、3、5年无病生存率和总体生存率分别为94.0%、78.5%、62.1%和98.0%、93.5%、90.6%。治疗前肿瘤组织中PARP-1高表达24例,低表达26例。PARP-1高表达组术后转移复发率为22.0%,明显高于低表达组的4.0%(P<0.05);高、低表达组1、2、3、4年无病生存率分别为87.5%、64.5%、55.3%、50.2%和100%、98.0%、98.0%、98.0%,差异均有统计学意义(均P<0.05)。结论治疗前PARP-1高表达,提示局部进展期直肠癌标准治疗后临床转归较差。Objective To assess the relationship between the expression of poly(ADP-ribose)polymerase-1(PARP-1)and clinical outcome of patients with locally advanced rectal cancer.Methods Fifty patients with locally advanced rectal cancer admitted in Sir Run Run Shaw Hospital Affiliated to Zhejiang University School of Medicine from January 2008 to November 2012 were enrolled in the study.The expression of PARP-1 in preoperative biopsy specimen was detected with immunohistochemistry method.All patients received preoperative neoadjuvant chemoradiotherapy,total mesorectal excision and postoperative adjuvant chemotherapy.The recurrence,metastasis and survival of patients with high and low expression of PARP-1 were compared.Re s ults Patients were followed up for average 47.85 months,and 13 cases had metastasis or recurrence(26.0%).The median of disease-free survival time was 46.75 months,and the 1-,3-,5-year disease-free survival rate and overall survival rate were 94.0%,78.5%,62.1%and 98.0%,93.5%,90.6%respectively.There were 24 cases with high expression of PARP-1 before treatment and 26 cases with low expression of PARP-1.The 1-,2-,3-,and 4-year disease-free survival rates of the high and low expression groups were 87.5%,64.5%,55.3%,50.2%and 100%,98.0%,98.0%and 98.0%respectively(P<0.05).Conclusion The high expression of PARP-1 before treatment indicates poor clinical outcome in patients with locally advanced rectal cancer.
