摘　　要：目的探讨肝动脉化疗栓塞术(TACE)联合选择性门静脉栓塞(SPVE)在肝癌二期切除术中的应用效果及对残留肝体积的影响。方法回顾性分析2017年7月1日至2018年12月31日于我院实施TACE后行肝癌二期切除手术患者(n=30,对照组)与实施TACE联合SPVE后行肝癌二期切除手术患者(n=30,观察组)的临床资料。比较两组的治疗效果。结果观察组左肝、右肝切除体积均小于对照组,术后残余左肝、右肝体积均大于对照组(P<0.05)。治疗后,两组的AST、ALT水平均较治疗前显著降低,且观察组低于对照组(P<0.05)。结论肝癌二期切除手术前实施TACE联合SPVE治疗,可最大限度保留肝体积,改善肝功能,对凝血功能损伤少,有利于提高患者存活率。Objective To investigate the application effect of transcatheter arterial chemoembolization(TACE)combined with selective portal vein embolization(SPVE)in secondary resection of liver cancer and its influence on residual liver volume.Methods The clinical data of patients who underwent TACE before secondary resection of liver cancer(n=30,control group)and patients who underwent TACE combined with SPVE(n=30,observation group)before secondary resection of liver cancer in our hospital from July 1,2017 to December 31,2018 were retrospectively analyzed.The therapeutic effects of the two groups were compared.Results Resected volumes of left liver and right liver in the observation group were less than those in the control group,and postoperative residual volumes of left liver and right liver in the observation group were greater than those in the control group(P<0.05).After treatment,AST and ALT levels in both groups were significantly lower than those before treatment,and those in observation group were lower than control group(P<0.05).Conclusion TACE combined with SPVE treatment before secondary resection of liver cancer can maximum retention the liver volume,improve liver function,has less damage on coagulation function,and is beneficial to improve the survival rate of patients.
