小切口外痔剥离加内痔结扎术治疗环状混合痔的效果及对患者肛门功能的影响

摘　　要：目的观察小切口外痔剥离加内痔结扎术治疗环状混合痔的效果及对患者肛门功能的影响。方法选取2018年4月至2019年11月我院收治的76例环状混合痔患者作为研究对象,将其随机分为对照组(38例,常规外痔剥离加内痔结扎术)和观察组(38例,小切口外痔剥离加内痔结扎术)。比较两组治疗效果。结果观察组治疗总有效率显著高于对照组,术后首次排便时间、创面愈合时间均短于对照组,术后7 d疼痛程度评分低于对照组(P<0.05)。术后12、24 h,两组ACTH、COR水平均升高,但观察组升高程度显著低于对照组(P<0.05)。术后1个月,两组Wexner评分均降低,GQOL-74评分均升高,且观察组优于对照组(P<0.05)。结论小切口外痔剥离加内痔结扎术治疗环状混合痔能够显著减轻患者术后疼痛和应激反应,提高临床治愈率,缩短术后首次排便时间和创面愈合时间,且患者肛门功能恢复效果好,生活质量较高。Objective To observe the effect of small incision external hemorrhoid stripping and internal hemorrhoid ligation in the treatment of circular mixed hemorrhoids and its influence on anal function of patients.Methods Seventy-six patients with circular mixed hemorrhoids admitted in our hospital from April 2018 to November 2019 were selected as the study objects,and they were randomly divided into control group(38 cases,routine external hemorrhoids stripping and internal hemorrhoids ligation)and observation group(38 cases,small incision external hemorrhoids stripping and internal hemorrhoids ligation).The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group,the first defecation time after operation and wound healing time in the observation group were shorter than those in the control group,and the score of pain degree in the 7 d after operation in the observation group was lower than that in the control group(P<0.05).At 12,24 h after operation,the levels of ACTH and COR in the two groups increased,but the increasing degree in the observation group were significantly lower than those in the control group(P<0.05).One month after operation,the Wexner score of the two groups decreased,the GQOL-74 scores in the two groups increased,and those in the observation group were better than the control group(P<0.05).Conclusion Small incision external hemorrhoid stripping and internal hemorrhoid ligation can significantly reduce the postoperative pain and stress response of patients with circular mixed hemorrhoids,improve the clinical cure rate,shorten the first defecation time after operation and wound healing time,and the patients has good anal function recovery effect and high quality of life.
