低剂量螺旋CT联合7种肺癌自身抗体检测对早期肺癌的筛查价值

摘　　要：目的探讨低剂量螺旋CT联合7种肺癌自身抗体[p53、蛋白基因产物9.5(PGP9.5)、SOX区域Y相关HMG蛋白家族成员2(SOX2)、G抗原7(GAGE7)、RNA解旋酶自身抗体4-5(GBU4-5)、黑色素瘤抗原A1(MAGEA1)、肿瘤相关基因(CAGE)]检测对早期肺癌的筛查价值。方法对108例患者进行低剂量螺旋CT扫描并联合7种肺癌自身抗体检测,低剂量螺旋CT均未发现肺部病灶的患者38例(作为对照组),其余70例患者依据病理检查结果分为良性病变组(n=30)和肺癌组(n=40)。比较3组受试者血清7种肺癌自身抗体水平和阳性检出率,分析7种肺癌自身抗体单独或联合检测对肺癌的诊断价值。结果肺癌组、良性病变组和对照组受试者血清7种肺癌自身抗体水平比较,差异均有统计学意义(P﹤0.05);两两比较结果显示,肺癌组和良性病变组患者上述7种肺癌自身抗体水平均高于对照组受试者,且肺癌组患者7种肺癌自身抗体水平均高于良性病变组患者,差异均有统计学意义(P﹤0.05)。7种肺癌自身抗体联合检测诊断肺癌的灵敏度最高,为80.0%,诊断良性病变的灵敏度次之,为46.7%,但单一抗体的检测灵敏度较低。7种肺癌自身抗体联合检测共检出48例阳性,其中肺癌组阳性检出率最高,为66.7%。病理学检查结果显示,早期肺癌17例,7种肺癌自身抗体检测诊断早期肺癌14例,阳性检出率为82.4%。结论低剂量螺旋CT联合7种肺癌自身抗体检测诊断肺癌的灵敏度和特异度均较高,对肺癌的早期筛查及诊断有重要价值。Objective To explore the value of low-dose spiral CT combined with seven lung cancer autoantibodies[p53,protein gene product 9.5(PGP9.5),SRY-box containing gene 2(SOX2),G antigen 7(GAGE7),RNA helicase autoantibodies(GBU4-5),melanoma antigen A1(MAGEA1),and cancer associated gene(CAGE)]in the screening of early lung cancer.Method A total of 108 patients received low-dose spiral CT scan combined with 7 kinds of autoantibodies detection.38 patients undergoing low-dose spiral CT without detection of any lung lesions were included into the control group,and the remaining 70 patients were divided into benign lesion group(n=30)and lung cancer group(n=40)according to pathological results.The serum levels of seven lung cancer autoantibodies and the positive detection rate of the 3 groups were compared,and the diagnostic value of the seven lung cancer autoantibodies alone or in combination was analyzed.Result The comparison of serum levels of seven lung cancer autoantibodies in the lung cancer group,benign disease group and control group was statistically significant(P<0.05).Pairwise comparison results show that the levels of the seven lung cancer autoantibodies in the lung cancer group and the benign disease group were higher than those in the control group,and the seven lung cancer autoantibodies in the lung cancer group are higher than those in the benign disease group,and the differences were statistically significant(P<0.05).The 7 lung cancer autoantibodies have the highest sensitivity in detecting lung cancer at 80.0%,followed by the sensitivity of detecting benign lesions at 46.7%,but the detection sensitivity of single antibody was low.48 cases were detected by combined 7 kinds of lung cancer autoantibodies with the highest positive detection rate in lung cancer group(66.7%).Pathological examination showed 17 cases of early lung cancer,and 14 cases of early lung cancer were detected by 7 kinds of lung cancer autoantibodies.The positive detection rate was 82.4%.Conclusion low-dose spiral CT combined with 7 kinds of l
