摘  要:目的探讨胃癌患者胃黏膜上皮组织中DNA甲基化转移酶3A(DNMT3A)表达与幽门螺杆菌(Hp)感染的关系。方法选取胃癌(GC)、腺瘤性息肉(GA)、慢性萎缩性胃炎(CAG)、慢性非萎缩性胃炎(CNAG)患者各30例,比较不同患者胃黏膜组织中DNMT3A mRNA的表达水平及Hp的感染情况。结果4种胃部疾病患者胃黏膜组织中DNMT3A mRNA的相对表达量比较,差异有统计学意义(P﹤0.01)。随着病情的发展,DNMT3A mRNA的相对表达量呈逐渐升高趋势。GC患者胃黏膜组织中DNMT3A mRNA的相对表达量高于CAG和CNAG患者,GA患者胃黏膜组织中DNMT3A mRNA的相对表达量高于CNAG患者(P﹤0.05)。在同种疾病的胃黏膜组织中,Hp感染胃部疾病患者胃黏膜组织中DNMT3A mRNA的相对表达量高于非Hp感染的胃部疾病患者(P﹤0.05)。低分化、伴有淋巴结转移GC患者GC组织中DNMT3A mRNA的相对表达量分别高于高中分化、不伴淋巴结转移的GC患者(P﹤0.05)。结论Hp感染可诱导胃黏膜中DNMT3A高表达,DNMT3A表达的上调可能促进GA、GC的发生发展,Hp感染和DNMT3A可协同作用参与从慢性炎症向GC发展的过程。Objective To investigate the relationship between the expression of DNA methyltransferase 3 alpha(DNMT3A)and helicobacter pylori(Hp)infection in the gastric mucosal epithelium tissues of patients with gastric cancer.Method Thirty cases of each disease including gastric cancer(GC),adenomatous polyps(GA),chronic atrophic gastritis(CAG)and chronic non-atrophic gastritis(CNAG)were selected.The expression levels of DNMT3A mRNA and Hp infection in gastric mucosal tissues of different patients were compared.Result The relative expression of DNMT3A mRNA in gastric mucosal tissues of the four groups of gastric disease patients were compared,and the difference was statistically significant(P<0.01).With the development of the disease,the relative expression of DNMT3A mRNA showed a gradually increasing tendency.The relative expression of DNMT3A mRNA in gastric mucosal tissue of GC patients was higher than that of CAG and CNAG patients,the relative expression of DNMT3A mRNA in gastric mucosal tissue of GA patients was higher than that of CNAG patients,and the difference was statistically significant(P<0.05).In gastric mucosal tissues of the same disease,the relative expression of DNMT3A mRNA in gastric mucosal tissues of patients with Hp infection was higher than that of patients without Hp infection,and the difference was statistically significant(P<0.05).The relative expression of DNMT3A mRNA in GC tissues of poorly differentiated GC patients with lymph node metastasis was higher than that in highly differentiated GC patients without lymph node metastasis(P<0.05).Conclusion Hp infection can induce the high expression of DNMT3A in gastric mucosa.The up-regulation of DNMT3A expression may promote the occurrence and development of GA and GC.Hp infection and DNMT3A have a synergistic effect and participate in the development process from chronic inflammation to GC.
