摘 要:目的本研究拟通过检测肿瘤癌痛患者外周血中相关致痛因子的表达水平,探讨关键致痛因子的含量及变化规律与癌性疼痛的相关性。方法收集新华医院崇明分院2014年7月至2015年4月收住入院的伴癌无痛患者(A组)53例,肿瘤轻度疼痛患者(B组)53例,肿瘤中重度疼痛患者(C组)25例,应用蛋白芯片分次测定3组患者及中重度疼痛患者抗癌痛治疗后(D组)血清中致痛因子的表达水平。结果运用人类细胞因子抗体芯片Human Cytokine Array C59检测4组患者血浆内疼痛相关的细胞因子变化。运用Volcano Plot对4组芯片检测结果进行两两比较,检测差异性的细胞因子。采用SPSS 18.0统计分析差异蛋白在4个时期的表达变化趋势。采用聚类分析检测差异性的细胞因子,探测样本中相似蛋白的表达谱,对目的因子功能进一步的判别和分类。结论SPSS 18.0统计分析结合聚类分析结果,提示IL-2、IL-7、TNF-β、MIF、GM-CSF和TGF-β1细胞因子在癌痛维持和加重过程中扮演重要角色,IL-2、MCP-4、Ckβ8-1和Fractalkine细胞因子对癌痛减缓具有重要作用。实验结果发现,免疫调节因子IL-2在癌痛的维持、加重以及减缓中均发生了显著改变。研究结果提示,通过改善机体的免疫治疗状态,提高机体整体免疫功能,可能有效的缓解癌痛,提高顽固性癌痛缓解率。Objective:The purpose of this study was to investigate the changes of relative pain factor in peripheral blood and its expression level,also to explore the relationship between the serum cytokine level and cancer pain.Methods:We conducted the study in Xinhua Chongming hospital between April 2014 and July 2015.The enrolled participants were divived into three groups as 53 painless patients in Group A,53 mild pain admitted patients in Group B and 25 severe pain admitted patients in Group C.The changes of cytokines in cancer pain patients were detected by protein-chip technique.Futhermore,we compared the differences of cytokines in pairs with Volcano Plot Assay.The tendency of cytokines changes were analysed by SPSS 18.0 statistic system method and the cancer pain-associated cytokines were classified by cluster analysis.Results:We examined the changes of cytokines in cancer patients in 4 groups by Human Cytokine Array C59 protein-chip.The differences of cytokines were compared in pairs by Volcano Plot Assay.Besides,the variation trend of cytokines in different groups were counted by SPSS 18.0 statistical system.To provide further evidence,we classified the cancer pain-associated cytokines and explored the function of the cytokines by cluster analysis.Conclusion:Statistically,the result showed that IL-2,IL-7,TNF-β,MIF,GM-CSF and TGF-β1 play important roles in the maintenance and deterioration of cancer pain while IL-2、MCP-4、Ckβ8-1and Fractalkine involved in the process of pain relief.However,it is worth mentioning that IL-2 involved in not only the pain maintenance and pain deterionration but also the pain relief.These results indicated that increasing the immune functions of the body during therapy is possibly an effective means of relieving cancer pain and then improve the remission rate of intractable cancer pain.
