摘  要:目的探讨结直肠癌患者根治术前营养水平对术后恢复及并发症发生率的影响.方法将70例结直肠癌患者按营养风险筛查方法分为营养正常组(40例)和营养不良组(30例).比较两组患者肛门排气时间、切口愈合时间、住院时间、并发症发生情况.比较手术前后两组C反应蛋白、白介素-6、降钙素原、肿瘤坏死因子、白蛋白、前白蛋白、转铁蛋白水平.结果营养正常组肛门排气时间、切口愈合时间及住院时间均显著短于营养不良组(P<0.01).术后7 d两组C反应蛋白、白介素-6、降钙素原、肿瘤坏死因子水平均较术前显著升高(P<0.01),营养正常组显著低于营养不良组(P<0.01).术前及术后7 d营养正常组白蛋白、前白蛋白、转铁蛋白水平均显著高于营养不良组(P<0.01),术后7 d营养不良组白蛋白、前白蛋白、转铁蛋白水平显著高于术前(P<0.01).营养正常组并发症发生率显著低于营养不良组(P<0.05).结论结直肠癌术前营养不良患者术后肛门排气时间、切口愈合时间及住院时间均较长,更容易出现炎症反应,并发症发生率较高,临床应给予适当干预措施.Objective To investigate the effect of preoperative nutritional level on wound infection after radical operation in patients with colorectal cancer.Methods 70 cases of colorectal cancer were divided into normal nutrition group(40 cases)and dystrophy group(30 cases)according to the nutritional risk screening method(NRS2002).Anal exhaust time,incision healing time,hospitalization time,and complications in both groups were compared.The levels of C-reactive protein,interleukin-6,procalcitonin,tumor necrosis factor,albumin,prealbumin and transferrin in the two groups were compared before and after surgery.Results The anal exhaust time,incision healing time and hospital stay were significantly shorter in the normal nutrition group than in the dystrophy group(P<0.01).At 7 days after operation,the levels of C-reactive protein,interleukin-6,procalcitonin and tumor necrosis factor were significantly higher than those before operation(P<0.01),and the normal nutrition group was significantly lower than the dystrophy group(P<0.01).The levels o£albumin,prealbumin and transferrin in the normal nutrition group were significantly higher than those in the dystrophy group before and 7 days after surgery(P<0.01).After 7 days,the levels of albumin,prealbumin and transferrin in the dystrophy group were significantly higher than those before surgery(P<0.01).The incidence of complications in the normal nutrition group was significantly lower than that in the dystrophy group(P<0.05).Conclusions Patients with preoperative malnutrition of colorectal cancer have longer anal exhaust time,incision healing time and hospitalization time,are more prone to inflammatory reaction,and have a higher incidence of complications.Clinical intervention should be given.
