延髓梗死与急性期失眠相关性分析

摘　　要：目的探讨缺血性卒中患者中延髓梗死与急性期失眠的相关性。方法连续纳入急性缺血性卒中患者,收集临床资料,包括患者影像学、人口学及相关量表等。根据影像结果登记梗死部位;根据相关睡眠障碍标准定义失眠。二分类logisti回归模型分析延髓粳死对失眠的影响。结果321例缺血性卒中患者,急性期失眠135例(42.1%),脑干梗死77例(24.0%),延髓梗死21例(6.5%)。与非失眠患者比较,急性期失眠患者脑干粳死及延髓梗死比例更高;Logisic回归分析表明,延髓梗死患者发生急性期失眠风险比非失眠者高4.325倍(P=0.021,OR=5.325,95%CI 1.282-22.121)。结论约40%急性缺血性卒中患者发生急性期失眠延髓梗死可能增加急性期失眠发生风险。Objective To investigate the impact of medullary infarction on insomnia in patients with acute ischemic stroke.Methods Patients diagnosed with acute ischemic stroke were consecutively screened.The radiologic findings,demographic data,and scores of different scales were collected and analyzed.Infarction locations were recorded according to radiologic images.Insomnia was diagnosed by sleep-disorder diagnostic criteria.Logistic regression model was applied to analyzed the effect of medullary infarction on insomnia.Results Three hundred and twenty-one acute ischemic stroke patients were enrolled and 135 patients(42.1%)suffered insomnia in acute phase.Seventy-seven patients(24.0%)got brain stem infarction while 21 patients(6.5%)got medullary infarction.Compared with non-insomnia patients,insomnia patients had higher proportion of brain stem infarction and medullary infarction.Multivariate regression analysis showed that the odds ratio(OR)of insomnia in acute medullary infarction patients increased by 4.325 times(P=0.O21,OR=5.325,95%Cl 1.282-22.121).Conclusion Almost forty percentage patients diagnosed with acute ischemic stroke experiences insomnia.Acute medullary ischemic stroke is likely to increase the risk of insomnia.
