甲状腺全切术治疗甲状腺微小癌的应用

摘　　要：目的探讨甲状腺微小癌患者采用甲状腺全切术治疗的应用效果。方法选择本院于2018年4月至2019年4月收治的150例甲状腺微小癌患者作为研究对象,按照手术治疗方法不同分为对照组(70例,采用甲状腺腺叶全切术治疗),观察组(80例,采用甲状腺全切术治疗),收集分析两组患者甲状腺治疗、临床指标、并发症、复发率以及2年生存率情况。结果治疗前两组患者T3、T4、TSH水平差异无统计学意义(P>0.05),治疗后观察组患者T3、T4、TSH水平均明显低于对照组,手术治疗时间、术后出血量均明显低于对照组,并发症发生率以及复发率明显低于对照组,2年生存率明显高于对照组,差异均有统计学意义(P<0.05)。结论两种手术治疗方法均能起到一定的治疗效果,但甲状腺全切术治疗,能够降低术中出血量,缩短手术治疗时间,降低术后并发症情况,提高患者预后质量,相较于甲状腺腺叶全切术具有更高的治疗效果。但术后患者容易出现不同程度的甲状腺功能损害,需要临床密切观察患者甲状腺功能指标,并给予对应治疗护理措施,改善患者预后质量。Objective To investigate the effect of total thyroidectomy in the treatment of thyroid microcarcinoma. Methods One hundred and fifty patients with thyroid microcarcinoma in our hospital from April 2018 to April 2019 were selected and divided into control group(70 cases, treated with total thyroidectomy) and observation group(80 cases, treated with thyroid lobectomy) according to different surgical treatment methods. Thyroid function indexes, clinical indexes, complications, and complications of two groups before and after treatment were collected and analyzed Recurrence rate and twoyear survival rate. Results Before treatment, there was no significant difference in T3, T4 and TSH levels between the two groups(P>0.05). After treatment, the levels of T3, T4 and TSH in the observation group were significantly lower than those in the control group, the operation time and postoperative bleeding volume were significantly lower than those in the control group, the incidence of complications was significantly lower than that of the control group, the recurrence rate was significantly lower than that of the control group, and the two-year survival rate was significantly higher than that of the control group, statistical significance(P<0.05). Conclusion The two surgical treatment methods can play a certain therapeutic effect, but total thyroidectomy can reduce intraoperative blood loss, shorten the operation time, reduce postoperative complications, improve the quality of prognosis of patients, compared with thyroid lobectomy, it has higher therapeutic effect. However, patients are prone to varying degrees of thyroid function damage after surgery. It is necessary to closely observe the thyroid function indexes of patients and give corresponding treatment and nursing measures to improve the prognosis quality of patients.
