两种术式(电视胸腔镜辅助小切口与传统开胸)肺叶切除术的临床对比研究

摘　　要：目的:对比分析电视胸腔镜辅助小切口肺叶切除术与传统开胸肺叶切除术两种术式的临床效果及安全性,为临床医师选择合理术式提供参考。方法:选择2017年4月至2019年11月康平县人民医院收治的70例肺癌患者为研究对象,根据患者意愿将其分为两组,对照组和观察组,两组样本量均为35例。对照组行传统开胸手术,观察组行电视胸腔镜辅助小切口手术。对比两组患者的手术相关指标、术后24h内疼痛评分及并发症发生率。结果:两组患者的手术时间组间对比结果无统计学差异(P>0.05);观察组患者的手术失血量、术后引流及住院时间较对照组均明显减少,且观察组术后24h疼痛评分及术后并发症发生率均明显低于对照组,P<0.05。结论:电视胸腔镜辅助小切口术式在肺叶切除中的应用,可在减少患者术中医源性损伤的基础上,提升手术预后安全性及患者术后恢复质量,疗效显著。Objective:To compare the clinical effects and safety of video-assisted thoracoscopic small incision lobectomy and traditional thoracotomy lobectomy,so as to provide reference for clinicians to choose reasonable surgical methods.Methods:A total of 70 cases with lung cancer in Kangping People's Hospital from April 2017 to November 2019 were selected as the research objects,and they were divided into two groups according to the wishes of the patients,the control group and the observation group,the sample size of both groups were 35 cases.The control group received traditional thoracotomy,and the observation group received video-assisted thoracoscopic surgery.The operation related indexes,postoperative pain score within 24 hours and the incidence of complications were compared between the two groups.Results:There was no significant difference in the operation duration between the two groups(P>0.05);the operation blood loss volume,postoperative drainage and hospitalization duration of the observation group were significantly lower than those of the control group,and the postoperative 24-hour pain score and the incidence of postoperative complications in the observation group were significantly lower than those in the control group,P<0.05.Conclusion:The application of video-assisted thoracoscopic small incision in lobectomy can improve the safety of surgical prognosis and the quality of postoperative recovery on the basis of reducing the iatrogenic injury of patients.
