用三种筛查方法对孕妇进行唐氏综合征产前筛查的效果比较

摘　　要：目的:比较用血清学三联筛查法、无创DNA检测法和超声检查法对孕妇进行唐氏综合征产前筛查的效果。方法:将2018年2月至2019年12月在广东省第二人民医院进行产前检查的孕妇2770例作为研究对象。根据孕妇对唐氏综合征产前筛查方法选择的不同将其分为血清组、超声组和无创DNA组。为血清组孕妇采用血清学三联筛查法筛查唐氏综合征,为超声组孕妇采用超声检查法筛查唐氏综合征,为无创DNA组孕妇采用无创DNA检测法筛查唐氏综合征,然后比较三组孕妇唐氏综合征产前筛查结果的准确率、假阳性率和假阴性率。结果:与血清组孕妇相比,超声组孕妇唐氏综合征产前筛查结果的准确率较高,假阳性率和假阴性率均较低,P<0.05。与血清组孕妇和超声组孕妇相比,无创DNA组孕妇唐氏综合征产前筛查结果的准确率较高,假阳性率和假阴性率均较低,P<0.05。结论:用无创DNA检测法对孕妇进行唐氏综合征产前筛查的效果优于血清学三联筛查法和超声检查法,用超声检查法对孕妇进行唐氏综合征产前筛查的效果优于血清学三联筛查法。Objective:To compare the effects of prenatal screening for Down syndrome in pregnant women with serological triple screening,non-invasive DNA testing and ultrasound.METHODS:A total of 2,770 pregnant women who underwent prenatal examinations in the Second People's Hospital of Guangdong Province from February 2018 to December 2019 were included in the study.Pregnant women were divided into serum group,ultrasound group and non-invasive DNA group according to their different selection of prenatal screening methods for Down syndrome.For groups of pregnant women serum screening for down syndrome by three serologic screening method,for the ultrasound group of pregnant women with ultrasonic test screening for down syndrome,for noninvasive DNA group of pregnant women with noninvasive DNA test screening for down syndrome,then compared three groups of pregnant women down's syndrome prenatal screening result accuracy,positive rate and negative rate.Results:Compared with the serum group,the accuracy of prenatal screening results of pregnant women with Down syndrome in the ultrasound group was higher,and the positive rate and negative rate were lower,P<0.05.Compared with the serum group and the ultrasound group,the accuracy of prenatal screening results in the noninvasive DNA group was higher,and the positive rate and negative rate were lower(P<0.05).Conclusion:The effect of noninvasive DNA test on pregnant women with Down syndrome prenatal screening is better than that of serological triple screening and ultrasonography,and the effect of ultrasonography on pregnant women with Down syndrome prenatal screening is better than that of serological triple screening.
