摘 要:目的探索提高肺癌循环肿瘤细胞(circulating tumor cells,CTC)阳性率的策略。方法对病理确诊肺癌患者46例分别采取外周动脉血(A组)和外周静脉血(V组)各5 mL,采用免疫磁珠阴性富集联合免疫荧光原位杂交法对CTC进行富集计数和鉴定,计算其阳性率。结果A组CTC含量(1.98±1.73[个/3.2 mL])明显高于V组(1.39±1.86[个/3.2 mL])(t=2.533,P=0.015)。A组的CTC检出率(71.7%)明显高于V组(52.2%)(χ^(2)=8.873,P=0.032)。在早期(1期)肺癌,A组的CTC阳性率(50.0%)明显高于V组(14.3%)(χ^(2)=8.857,P=0.013)。结论肺癌患者外周动脉血CTC含量明显高于外周静脉血;检测外周动脉血CTC相比外周静脉血,可获更高阳性率,更有利于肺癌的早期诊疗及随访。Objective To explore strategies to improve the positive rate of circulating tumor cells(CTC)in lung cancer.Methods It was sampled that 5 ml respectively of peripheral arterial blood(Group A)and peripheral venous blood(Group V)of 46 patients with pathologically confirmed lung cancer,and the enrichment,count and identification of CTC were performed by immunomagnetic bead negative enrichment combined with immunofluorescence in situ hybridization,and the CTC positive rate was calculated.Results The CTC content in the group A(1.98±1.73[pieces/3.2 ml])was significantly higher than that in the group V(1.39±1.86[pieces/3.2 ml])(t=2.533,P=0.015).The CTC detection rate in the group A(71.7%)was significantly higher than that in the group V(52.2%)(Chi-square value=8.873,P=0.032).In early stage(first stage)of lung cancer,the positive rate of CTC in the group A(50.0%)was significantly higher than that in the group V(14.3%)(chi-square value=8.857,P=0.013).Conclusion The content of CTC in peripheral arterial blood is significantly higher than that in peripheral venous blood.Compared with peripheral venous blood,peripheral arterial blood CTC detection can obtain a higher positive rate,which is more conducive to early diagnosis,treatment and follow-up of lung cancer.
