针刺络穴对卒中恢复期患者心率变异性频域指标的影响

摘　　要：目的观察针刺络穴对脑卒中恢复期患者心率变异性中短程频域指标的影响,探讨自主神经在针刺中的变化规律。方法将60例脑卒中恢复期患者随机分为大钟穴组和通里组,每组30例,分别针刺大钟穴和通里穴。用medilogAR12心电记录仪的短程记录,检测两组患者在针刺前5 min、针刺时、留针10 min、留针20 min后出针及出针后5 min的频域指标[低频功率(LF)、高频功率(HF)、低频高频之比(LF/HF)],经对数转化后采用SPSS22.0统计软件分析比较两组lg LF、lg HF、lg LF/HF的差异。结果与针刺前相比各时间点lg LF、lg HF、lg LF/HF的值均存在差异(P<0.05);lg LF与lg HF的值有相同的时间效应趋势。结论针刺足少阴肾经之络大钟穴、手少阴心包经之络通里穴可调整卒中恢复期患者频域指标lg LF、lg HF、lg LF/HF的变化,且lg LF与lg HF呈相似的时间效应趋势。Objective:To observe the effect of acupuncture point on short-range frequency-domain index of heart rate variability in stroke patients during recovery period,and to explore the change rule of autonomic nerve in acupuncture.Methods:A total of 60 patients with convalescent stroke were randomly divided into the Dazhong point group and Tongli point group,30 cases in each group and received acupuncture on the respective point.The frequency-domain indexes(LF,HF,LF/HF)of the two groups were detected by short-range recording of medilogAR12 electrocardiograph at the first 5 min of acupuncture,10 min of acupuncture,20 min of acupuncture and the differences between the two groups in lg LF,lg HF,lg LF/HF were analyzed and compared using SPSS 22.0 statistical software.Results:The values of each time point lg LF,lg HF,lg LF/HF were different compared with those before acupuncture(P<0.05),and the values of lg LF and Tongli point had the same time effect trend.Conclusion:The frequency domain index of stroke recovery period lg LF,lg HF,lg LF/HF can be adjusted by acupuncture at Dazhong point and Tongli point,and the lg LF and lg HF show a similar trend of time effect.
