摘 要:目的:观察胶原抗菌功能性敷料在预防乳腺癌放疗患者2级放射性皮炎发生及降低皮肤异常感觉中的疗效。方法:将2017年1月至2018年9月收治的120例乳腺癌改良根治术后需要接受放疗的患者随机分为观察组、对照组和常规护理组。观察3组患者在放疗3周后及放疗结束后1周放射性皮炎的分级情况,以及各级放射性皮炎发生时间和放射部位皮肤的感觉变化。结果:放疗3周后及放疗结束后1周,观察组2级以上放射性皮炎发生率低于对照组和常规护理组(P<0.05);观察组患者的皮肤烧灼感、瘙痒、牵拉感的评分明显低于对照组和常规护理组(P<0.05),在刺痛感方面,各组差异无统计学意义(P=0.224);观察组出现2级放射性皮炎的平均时间明显低于对照组和常规护理组(P<0.05)。结论:胶原抗菌功能性敷料能够显著降低2级放射性皮炎的发生率,延迟2级放射性皮炎的出现时间,改善放疗后皮肤烧灼瘙痒及牵拉感,提高患者的生活质量及放疗的依从性。Objective:To investigate the effect of anti-bacterial collagen functional dressing on prevention of grade 2 radiation dermatitis and reduction of abnormal skin sensation in breast cancer patients with radiotherapy.Methods:A total of 120 patients who received radiotherapy after modified radical mastectomy for breast cancer were randomly divided into observation group,control group and routine nursing group.After 3 weeks of radiotherapy and 1 week after the end of radiotherapy,the grading and occurrence time of radioactive dermatitis,and the sensory changes of skin at the radiation site in the three groups were observed.Results:After 3 weeks of radiotherapy and 1 week after the end of radiotherapy,the incidence of grade≥2 radioactive dermatitis in the observation group was lower than those in the control group and the conventional nursing group(P<0.05).The scores of skin burning sensation,itching and tension in the observation group were significantly lower than those in the control group and the conventional nursing group(P<0.05),and there was no significant difference in tingling sensation(P=0.224).The mean occurrence time of grade 2 radioactive dermatitis in the observation group was significantly lower than that in the control group and the conventional nursing group(P<0.05).Conclusion:Application of anti-bacterial collagen functional dressing can significantly reduce the incidence of grade 2 radio⁃active dermatitis,delay the occurrence of grade 2 radioactive dermatitis,improve the symptoms of burning sensation,itching and tension after radiotherapy,and improve the quality of life and radiotherapy compliance of patients.
