喉返神经减压术对甲状腺手术时喉返神经损伤患者的治疗效果

摘　　要：目的对甲状腺手术时喉返神经损伤患者采用喉返神经减压术治疗的临床效果进行观察。方法以2017年3月至2018年5月我院收治的62例甲状腺手术时喉返神经损伤患者为研究对象,并对43例患者行喉返神经探查,对手术时喉返神经损伤的原因进行分析,采取相应喉返神经减压术进行治疗,作为观察组;其余19例患者行保守治疗,作为对照组。并对两组患者治疗前后的嗓音声学情况及喉返神经电生理学检查结果进行对比分析。结果观察组患者治疗后基频微扰、振幅微扰、标准化噪声能量与对照组相比均明显降低(P<0.05);且观察组患者经治疗后诱发电位与对照组相比明显升高(P<0.05)。结论对甲状腺手术时返喉神经损伤患者采用喉返神经探查,并结合喉返神经减压术治疗,可有效帮助患者实现声带运动功能的恢复,提高治疗效果。Objective Observe the clinical effect of decompression of the recurrent laryngeal nerve in patients with recurrent laryngeal nerve injury during thyroid surgery.Methods Taking 62 patients with recurrent laryngeal nerve injury during thyroid surgery admitted to our hospital from March 2017 to May 2018 as the research object,43 patients underwent recurrent laryngeal nerve exploration and analysis of the causes of recurrent laryngeal nerve injury during surgery,take the corresponding recurrent laryngeal nerve decompression surgery as the observation group;the remaining 19 patients underwent conservative treatment as the control group.The acoustic conditions of the voice and the electrophysiological examination results of the recurrent laryngeal nerve of the two groups of patients before and after treatment were compared and analyzed.Results The fundamental frequency perturbation,amplitude perturbation,and standardized noise energy of the observation group were significantly lower than those of the control group after treatment(P<0.05);and the observation group was significantly increased after treatment compared with the control group(P<0.05).Conclusion For patients with recurrent laryngeal nerve injury during thyroid surgery,the use of recurrent laryngeal nerve exploration combined with decompression of recurrent laryngeal nerve can effectively help patients achieve the recovery of vocal cord motor function and improve the treatment effect.
