管状胃代食管吻合术在食管癌根治术中的应用价值

摘　　要：目的探究管状胃代食管吻合术在食管癌根治术中的应用价值。方法选取102例食管癌患者为受试对象,随机数字表法分为观察组与对照组各51例。对照组予以传统全胃代食管吻合术治疗,观察组实施管状胃代食管吻合术进行治疗。比较围手术期指标及术后并发症发生率。结果经治疗,两组6个月内无死亡脱组案例,观察组胃肠减压量明显低于同期对照组(P<0.05);而手术时间、术中出血量、住院时间、术后下床活动时间、淋巴结清扫数比较无统计学意义(P>0.05);两组吻合口狭窄、胃排空障碍、肺部感染等并发症发生率无统计学意义(P>0.05),而观察组胃食管反流发生率明显更低(P<0.05)。结论管状胃代食管吻合术可明显改善食管癌患者症状,且术后胃食管反流发生率低。Objective To investigate the value of tubular gastroesophageal anastomosis in radical resection of esophageal cancer.Methods A total of 102 patients with esophageal cancer who were treated were enrolled.The random number table method was used to divide them into observation group and control group,51 cases each.The control group was treated with traditional total gastroesophageal anastomosis.The observation group was treated with tubular gastroesophageal anastomosis.Compare the perioperative indicators and postoperative complication rates.Results After treatment,there were no cases of death in the two groups.The gastrointestinal decompression volume of the observation group was significantly lower than that of the control group(P<0.05).There was no statistically significant difference in operation time,intraoperative blood loss,hospital stay,postoperative activity time,and lymph node dissection(P>0.05).There was no significant difference in the incidence of complication of anastomotic stenosis,gastric emptying,and pulmonary infection between the two groups(P>0.05),while the incidence of gastroesophageal reflux was significantly lower in the observation group(P<0.05).Conclusion Tubular gastroesophageal anastomosis can significantly improve the symptoms of esophageal cancer patients,and the incidence of postoperative gastroesophageal reflux is low.
