摘　　要：目的:探讨不同封闭方法对紫杉醇外渗性皮肤损伤结局的影响。方法:选取30只健康新西兰大白兔,分别在左侧肋腹部前端、后端、右侧肋腹部前端、后端皮下注射紫杉醇注射液1 ml建立紫杉醇外渗动物模型。模型建立5 min后,距注射点5 mm环形注射生理盐水、低剂量地塞米松+利多卡因、高剂量地塞米松+利多卡因封闭处理。封闭处理后于Day1、Day3、Day7、Day14、Day21时间点进行皮肤取样,病理染色检测不同处理组皮肤样本的炎细胞浸润、皮下组织坏死及毛细血管增生情况,比较不同封闭方法处理紫杉醇外渗的效果。结果:自然观察组皮下组织坏死程度最明显,生理盐水封闭液组炎细胞浸润程度最明显,地塞米松+利多卡因封闭液组皮肤损伤较少,且高剂量组的毛细血管增生程度最明显。结论:高剂量地塞米松+利多卡因封闭液可以明显促进组织修复,减轻紫杉醇外渗对皮肤造成的损伤。Objective:To explore the effects of different blocking methods on the paclitaxel-induced extravasation skin injury.Methods:An animal model of paclitaxel extravasation was established by subcutaneous injection of 1 ml paclitaxel at the front and rear on each side of the left and right costal abdomen in 30 healthy New Zealand White rabbits.The paclitaxel extravasation points were respectively treated with injection of the saline closure,low-dose dexamethasone combined with lidocaine,and high-dose dexamethasone combined with lidocaine around 5 mm,5 min after the model established.At the time points of Day1,Day3,Day7,Day14 and Day21 after blocking,the skin samples were detected by pathological analysis,comparing the inflammatory cell infiltration,subcutaneous tissue necrosis,and capillary hyperplasia.Results:The subcutaneous tissue necrosis was significantly obvious in natural observation group.The inflammatory cell infiltration was significantly obvious in saline closure group.The capillary hyperplasia was obvious in low-dose and high-dose dexamethasone combined with lidocaine group,and significantly obvious in high dose group.Conclusions:High dose dexamethasone combined with lidocaine can obviously promote tissue repair and alleviate the skin damage caused by paclitaxel extravasation.
