微小RNA-195与急性胰腺炎进展的相关性研究

摘　　要：目的:探讨miR-195与急性胰腺炎(Acute pancreatitis,AP)进展的相关性研究。方法:选择2018年1月至2019年12月在我院接受治疗的62例AP患者,依据轻重程度分为中/重度组和轻度组,另选同期体检的健康志愿者20例为对照组。所有人员入院当天采集静脉血,采用qRT-PCR检测血清中miR-195水平,分析miR-195与AP患者临床特征的相关性,并绘制ROC曲线分析其诊断AP的价值。结果:中/重度组、轻度组血清miR-195水平明显高于对照组(P<0.05),中/重度组血清miR-195水平明显高于轻度组(P<0.05)。血清miR-195与进行改良的CT严重指数(Modified CT severity index,MCTSI)评分、急性胰腺炎严重程度床边指数(Beside index of severity in acute pancreatitis,BISAP)评分、Ranson评分,急性生理与慢性健康评分II(Acute physiology and chronic health evaluation,APACHEII)相关(P<0.05),而与性别、年龄无相关性(P>0.05)。miR-195诊断AP的ROC曲线下面积为0.881,截断值为4.08,灵敏度为91.7%,特异度为86.2%。结论:miR-195水平能反应AP患者的严重程度,可以作为诊断AP严重程度的标记物。Objective:To explore the correlation between miR-195 and the progress of acute pancreatitis(AP).Methods:Sixty-two patients with AP who were treated in our hospital from January 2018 to December 2019 were selected.They were divided into moderate/severe group and mild group according to severity,and healthy volunteers who had physical examination during the same period were selected as the control group.All patients collected venous blood on the day of admission.The level of miR-195 in serum was detected by qRT-PCR.The correlation between miR-195 and the clinical characteristics of AP was analyzed,and ROC curve drawn to analyze the value of AP diagnosis.Results:Serum miR-195 levels in the moderate/severe and mild groups were significantly higher than those in the control group.Serum miR-195 levels in the moderate/severe group were significantly higher than those in the mild group(P<0.05).Serum miR-195 was correlated with modified CT severity index(MCTSI)score,beside index of severity in acute pancreatitis(BISAP)score,Ranson score,acute physiology and chronic health evaluation(APACHEII)score(P<0.05),but not correlated with gender and age(P>0.05).The area under the ROC curve of miR-195 diagnostic AP was 0.881,the cutoff value was 4.08,the sensitivity was 91.7%,and the specificity was 86.2%.Conclusion:The levels of miR-195 can reflect the severity of AP patients and can be used as a marker to diagnose the severity of AP.
