摘　　要：目的比较右美托咪定、布托啡诺、帕瑞昔布预防正颌手术术后躁动的效果。方法将择期行正颌手术并符合入组标准的150例患者按随机数字表法分为甲乙丙3组,每组50例。3组患者均常规全身麻醉,术前10 min,甲组静脉输入右美托咪定(药液浓度4 g·mL^(-1),以1 g·kg^(-1)剂量缓慢静注,输注时间不少于10 min),乙组静脉注射布托啡诺1 mg,丙组静脉注射帕瑞昔布40 mg。其他麻醉处理相同。记录并比较3组患者术后VAS疼痛评分、Ramsay镇静评分、Rss躁动评分及药物不良反应情况。结果3组VAS评分和Rasay镇静评分比较,差异无统计学意义(P>0.05);3组躁动情况均得以改善,且术后Rss躁动评分比较差异无统计学意义(P>0.05)。丙组不良反应发生率高于其他2组(P<0.05)。结论右美托咪定、布托啡诺、帕瑞昔布均能缓解正颌手术后躁动。Objective To compare the efficacies of dexmedetomidine,butorfenol and parecoxib in the prevention of agitation after orthognathic surgery.Methods A total of 150 patients who were scheduled for orthognathic surgery and met the inclusion criteria were divided into three groups,with 50 patients in each group.All the patients received routine general anesthesia.In group A,dexmedetomidine(4 g·mL^(-1))was administered intravenously at a dose of 1 g·kg^(-1)10 minutes before surgery,with an infusion time exceeding 10 minutes.Group B and C were intravenously injected with 1 mg butorphanol and 40 mg parecoxib,respectively.VAS pain score,Ramsay sedation score,Rss agitation score and adverse drug reactions were compared among the three groups.Results The agitation was improved in all the three groups.There were no significant differences in Rss agitation score,VAS score and Rasay sedation score among the three groups(P>0.05).The incidence of adverse reactions in group C was higher than that in the other two groups(P<0.05).Conclusion Dexmedetomidine,butorfenol and Parecoxib can relieve the agitation after orthognathic surgery.
