老年性白内障超声乳化联合植入多焦点人工晶状体术后的视觉质量分析

摘　　要：目的观察AcrySof IQ ReSTOR+3D非球面多焦点人工晶状体植入术后的视觉质量。方法选择单纯年龄相关性白内障患者34例(44眼),分为A组和B组,A组(16例/26眼)植入AcrySof IQ ReSTOR+3D非球面多焦点人工晶状体;B组(18例/18眼)植入AcrySof IQ非球面单焦点人工晶状体。术后观察两组裸眼及最佳矫正远、中、近视力,应用OPD-ScanⅢ光学视觉质量分析仪测量患者的视觉质量,选取并记录患者术后3个月时4 mm瞳孔直径下的全眼的总高阶像差(tHOA)、球差(SA)、总慧差(TC)、总三叶草差(TT)、调制传递函数(MTF)和斯特尔比率(SR)。测量A组的离焦曲线。结果术后3个月两组裸眼中视力、近视力比较,差异均有统计学意义(P﹤0.05);全眼tHOA值和TT值A组小于B组,差异均有统计学意义(P﹤0.05);两组在5~30 cpd空间频率的MTF值比较,差异有统计学意义(P<0.05);两组SR值比较差异有统计学意义(P﹤0.05)。结论AcrySof IQ ReSTOR+3D非球面多焦点人工晶状体可以给患者提供良好的远、中、近视力,提高生活质量。Objective To analyze the visual quality of AcrySof IQ ReSTOR+3D aspheric multifocal intraocular lens implantation.Methods A total of 34 patients(44 eyes)with age-related cataract were enrolled.The patients were divided into group A and group B.Group A(16 patients/26 eyes)was treated with AcrySof IQ ReSTOR+3D aspheric multifocal intraocular lens implantation,and Group B(18 patients/18 eyes)was treated with AcrySof IQ aspherical single-focal intraocular lens implantation.The visual acuity of the naked eyes and the best corrected distant,intermediate and near visual acuity in the two groups were examined after operation.OPD-Scan III optical visual mass spectrometer was used to evaluate the visual quality.The total high order aberration(tHOA),spherical aberration(SA),total comatic aberration(TC),total trefoil(TT),modulation transfer function(MTF),and strehl ratio(SR)were measured in the two groups at 3 months after operation.The defocus curve was measured in group A.Results There were significant differences in the intermediate and near visual acuity of naked eye between the two groups at 3 months after operation between the two groups(P<0.05).tHOA and TT were lower in group A than those in group B(P<0.05).There were significant differences in MTF between 5 and 30 cpd spatial frequencies between the two groups(P<0.05).There was significant difference in SR between the two groups(P<0.05).Conclusion AcrySof IQ ReSTOR+3D aspheric multifocal intraocular lens might provide patients with best distant,intermediate and near visual acuity,and improve the quality of life.
