胰腺癌根治术联合门静脉切除术治疗门静脉受累的胰腺癌患者的疗效分析

摘　　要：目的探究胰腺癌根治术联合门静脉切除术治疗门静脉受累胰腺癌的临床效果。方法选取时间为2018年7月—2019年9月,共随机选取该科室80例门静脉受累的胰腺癌患者进行此次研究,选用电脑随机数字法将研究对象分为两组,对比组40例,研究组40例,对比组进行姑息旁路手术,研究组进行胰腺癌根治术联合门静脉切除术治疗,分析两组的手术指标、临床疗效和并发症发生状况。结果研究组的手术时间为(304.6±91.6)min,对比组为(335.6±89.4)min,差异无统计学意义(t=0.863,P=0.157);研究组术中出血量为(1123.7±253.2)mL,对比组为(1469.5±227.3)mL,差异有统计学意义(t=11.927,P<0.001);研究组门静脉阻断时间为(20.1±4.3)min,对比组为(26.2±5.1)min,差异有统计学意义(t=5.738,P<0.001);研究组临床治疗总有效率为97.50%,对比组为80.00%,差异有统计学意义(χ^(2)=4.173,P=0.016);研究组并发症发生率为5.00%,对比组为22.50%,差异有统计学意义(χ^(2)=5.632,P=0.021)。结论对门静脉受累胰腺癌患者采用胰腺癌根治术联合门静脉切除术,能够较好地完成临床手术治疗,有效改善患者的症状,提升临床治疗效果,降低并发症的发生,在临床中实用价值显著,可广泛地推荐运用。Objective To explore the clinical effect of radical resection of pancreatic cancer combined with portal vein resection in the treatment of pancreatic cancer with portal vein involvement.Methods Case selection time was July 2018 to September 2019,a total of 80 patients with pancreatic cancer with portal vein involvement in the undergraduate room were randomly selected for this study.The computer random number method was used to divide the study subjects into two groups and 40 cases in the comparison group and 40 cases in the study group,palliative bypass surgery in the comparison group,radical pancreatic cancer combined with portal vein resection in the study group,and the surgical index,clinical efficacy,and complications of the two groups were analyzed.Results The operation time of the study group was(304.6±91.6)min,the comparison group was(335.6±89.4)min,the difference was not statistically significant(t=0.863,P=0.157);the intraoperative blood loss in the study group was(1123.7±253.2)mL,the comparison group was(1469.5±227.3)mL,the difference was statistically significant(t=11.927,P<0.001);the portal vein block time of the study group was(20.1±4.3)min,and the comparison group was(26.2±5.1)min,the difference was statistically significant(t=5.738,P<0.001);the total effective rate of clinical treatment in the study group was 97.50%,the comparison group was 80.00%,the difference was statistically significant(χ^(2)=4.173,P=0.016);the complication rate in the study group was 5.00%,the comparison group was 22.50%,the difference was statistically significant(χ^(2)=5.632,P=0.021).Conclusion Radical pancreatectomy combined with portal vein resection for patients with portal vein involved pancreatic cancer can better complete the clinical surgical treatment,effectively improve the patient symptoms,enhance clinical treatment effect,and reduce the occurrence of complications.It has significant clinical practical value,which can be widely recommended.
