阴道镜+LEEP刀在宫颈上皮内瘤变诊治中的临床应用

摘　　要：目的探析阴道镜与LEEP刀联合应用在宫颈上皮内瘤变诊治中的临床价值,为临床医师选择合理治疗方案提供参考。方法选择2016年5月至2018年5月经宫颈液基薄层细胞学检查确诊的宫颈上皮内瘤变患者89例为研究对象,按照随机数字分组法将其分为两组,即试验组(45例)和对照组(44例),对照组采用微波法治疗,试验组给予阴道镜+LEEP刀治疗,比较两组的临床疗效,同时对比分析阴道镜下宫颈活检的病理与LEEP术后病理结果。结果试验组术中出血量少于对照组,手术时间短于对照组,术后复发率低于对照组(P<0.05);阴道镜下宫颈活检的病理与LEEP术后病理诊断相符80例,相符率为89.88%,升级4例,占比为4.49%,降级5例,占比为5.62%。随访12个月,4例存在病变残留情况,发生率为4.49%,其中1例情况较为严重接受2次LEEP手术治疗,其余3例病变较轻,物理治疗后随访结果显示正常。结论针对宫颈上皮内瘤变患者,阴道镜+LEEP刀诊治效果理想,利于患者病情恢复,可为临床诊治工作提供科学指导。Objective To explore the clinical value of colposcopy combined with LEEP knife in the diagnosis and treatment of cervical intraepithelial neoplasia,and to provide reference for clinicians to choose reasonable treatment options.Methods Form May 2016 to May 2018,89 cases with cervical intraepithelial neoplasia confirmed by cervical fluid basic wave layer cytology were selected as the study subjects.They were divided into two groups according to random number grouping method,namely,the experimental group(45 cases)and the control group(44 cases).The control group was treated by microwave method;the experimental group was treated with colposcopy and LEEP knife.The clinical efficacy of the two groups was compared,and the pathological results of colposcopic neck biopsy and LEEP were compared and analyzed.Results The amount of bleeding during operation in the experimental group was less than that in the control group,the operation duration was shorter than that in the control group(P<0.05),the recurrence rate was lower than that in the control group(P<0.05).The pathological diagnosis of cervical biopsy under colposcopy was consistent with that of pathological diagnosis after LEEP in 80 cases,the coincidence rate was 89.88%,and the upgrade rate was 4.4%.9%of the patients were reduced to 5.62%.Following up for 12 months,there were four cases with residual lesions.The incidence was 4.49%.One case received two LEEP operations;the other three cases had mild lesions.The follow-up results after physical therapy showed that the lesions were normal.Conclusion For patients with cervical intraepithelial neoplasia,the diagnosis and treatment effect of colposcopy+LEEP knife is ideal,which is conducive to the recovery of the patient's condition and can provide scientific guidance for clinical diagnosis and treatment.
