国产与进口吉西他滨治疗Ⅲ b~Ⅳ期肺鳞状细胞癌的成本-效果比较

摘　　要：目的:比较国产与进口吉西他滨治疗Ⅲb~Ⅳ期肺鳞状细胞癌的成本-效果(C/E)。方法:回顾性分析2017年4月至2019年3月河南省肿瘤医院收治的129例Ⅲb~Ⅳ期肺鳞状细胞癌患者的临床资料,将其中采用国产吉西他滨联合顺铂治疗的66例列为国产组,采用进口吉西他滨联合顺铂治疗的63例列为进口组。比较两组总有效率、疾病控制率、毒副反应发生率、1年生存率,同时统计比较其治疗成本,实施C/E分析。结果:两组总有效率、疾病控制率、毒副反应发生率、1年生存率比较均差异无统计学意义(P>0.05);国产组化疗药费低于进口组,差异有统计学意义(P<0.05),两组辅助药费、临床检查费、护理费比较差异无统计学意义(P>0.05);国产组C/E为105.3,明显低于进口组135.6,差异有统计学意义(P<0.05)。结论:国产吉西他滨可达到与进口吉西他滨相同的治疗效果,且具有明显药物经济学优势,是安全、有效、经济的化疗药物,临床应用价值较高。Objective:To compare the cost-effectiveness(C/E)of domestic and imported gemcitabine in the treatment of stage Ⅲb-Ⅳlung squamous cell carcinoma.Methods:The clinical data of 129 patients with stage Ⅲb-Ⅳlung squamous cell carcinoma in Henan Cancer Hospital from April 2017 to March 2019 were retrospectively analyzed.Among them,66 cases were treated with domestic gemcitabine combined with cisplatin as the domestic group,and 63 cases with imported gemcitabine combined with cisplatin as the imported group.The total effective rate,disease control rate,incidence of toxic and side effects and 1-year survival rate were compared between the two groups,and the treatment cost was statistically compared.Results:There was no significant difference in the total effective rate,disease control rate,incidence of toxic and side effects and 1-year survival rate between the two groups(P>0.05);the cost of chemotherapy in the domestic group was lower than that in the imported group,the difference was statistically significant(P<0.05),and there was no significant difference in the auxiliary drug fee,clinical examination fee and nursing fee between the two groups(P>0.05);the C/E of the domestic group was 105.3,significantly lower than that of the imported group 135.6,the difference was statistically significant(P<0.05).Conclusion:Domestic gemcitabine can achieve the same therapeutic effect as imported gemcitabine,and has obvious pharmacoeconomic advantages.It is a safe,effective and economical chemotherapy drug with high clinical application value.
