抗白延年汤联合小剂量化疗治疗老年急性髓系白血病的临床研究

摘　　要：目的分析抗白延年汤联合小剂量化疗治疗老年急性髓系白血病的临床效果。方法48例老年急性髓系白血病患者,根据奇偶数法分为参照组和实验组,每组24例。参照组采用小剂量化疗,实验组采用抗白延年汤结合小剂量化疗。比较两组患者免疫功能指标、化疗骨髓抑制情况、血象恢复时间、不良反应发生情况。结果治疗14 d后,实验组患者CD8^(+)低于参照组,CD4^(+)、CD4^(+)/CD8^(+)高于参照组,差异具有统计学意义(P<0.05)。实验组白细胞计数最低值(1.23±0.34)×10^(9)/L、血小板计数最低值(13.50±2.04)×10^(9)/L均高于参照组的(0.64±0.22)×10^(9)/L、(7.23±1.02)×10^(9)/L,差异具有统计学意义(P<0.05);实验组患者从化疗开始至中性粒细胞绝对值>1.0×10^(9)/L的时间(13.10±4.22)d、从化疗开始至血小板计数>20.0×10^(9)/L的时间(12.00±4.25)d短于对照组的(19.12±5.03)、(17.24±5.36)d,差异具有统计学意义(P<0.05)。实验组患者不良反应发生率略低于参照组,但差异无统计学意义(P>0.05)。结论抗白延年汤结合小剂量化疗治疗老年急性髓系白血病的效果显著,临床可进一步推广运用。Objective To analyze the clinical effect of Kangbai Yannian Decoction combined with low dose chemotherapy in the treatment of elderly patients with acute myeloid leukemia.Methods A total of 48 elderly patients with acute myeloid leukemia were divided into reference group and experimental group according to odd-even number method,with 24 cases in each group.The reference group was treated with low dose chemotherapy,and the experimental group was treated with Kangbai Yannian Decoction combined with low dose chemotherapy.The immune function indexes,chemotherapy bone marrow suppression status,hemogram recovery time,and occurrence of adverse reactions were compared between the two groups.Results After 14 d of treatment,the CD8^(+)of t he experimental group was lower than that of the reference group,and CD4^(+)and CD4^(+)/CD8^(+)were higher than those of reference group.All the difference was statistically significant(P<0.05).The lowest value of white blood cell count and platelet count of the experimental group were(1.23±0.34)×10^(9)/L and(13.50±2.04)×10^(9)/L,which were all higher than(0.64±0.22)×10^(9)/L and(7.23±1.02)×10^(9)/L of the reference group,and the difference was statistically significant(P<0.05).The time from the start of chemotherapy to the absolute value of neutrophils>1.0×10^(9)/L(13.10±4.22)d and time from the start of chemotherapy to platelet count>20.0×10^(9)/L(12.00±4.25)d of the experimental group were shorter than(19.12±5.03)and(17.24±5.36)d of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions of the experimental group was a little lower than that of the reference group,and the difference was statistically significant(P>0.05).Conclusion Kangbai Yannian Decoction shows remarkable effect in the treatment of elderly acute myeloid leukemia,which can be further applied in clinic.
