益生菌四联疗法对幽门螺旋杆菌阳性老年胃溃疡的疗效分析

摘　　要：目的探讨益生菌联合标准抗幽门螺旋杆菌(Hp)“四联”药物在老年性胃溃疡治疗中抗Hp的增效作用及临床疗效及不良反应情况。方法选择2018年1月—2020年6月本院收治的79例老年胃溃疡患者作为研究对象,根据治疗方法不同进行分组,其中,采用标准抗Hp四联药物治疗6周的45例患者纳入对照组,在对照组方案基础上加用益生菌治疗的34例患者纳入研究组,疗程均为连续6周。分析比较两组患者的临床疗效、Hp清除率及药物不良反应。结果两组患者临床疗效满足重复测量资料“球对称”假设,行主体内效应方差分析显示:两组间疗效评分比较,差异有统计学意义(P<0.05)。研究组不良反应发生率为11.8%,低于对照组的35.6%,差异有统计学意义(P<0.05)。随访时间为治疗4周后,对照组失访1例,研究组Hp清除率为94.1%,高于对照组的77.3%,差异有统计学意义(P<0.05)。结论应用益生菌联合标准抗Hp四联药物方案增加了老年性胃溃疡患者的Hp清除率,明显改善了临床症状,并降低了药物相关不良反应,具有重要的临床实际应用价值,值得推广应用。Objective To analyze and evaluate the synergistic effect,clinical efficacy,and side effects of probiotics combined with standard anti-Helicobacter pylori(Hp)"quadruple"drugs in the treatment of senile gastric ulcers.Methods A retrospective analysis was carried out on 79 cases of elderly patients with stomach ulcers those admitted to Sir run run hospital from January 2018 to June 2020.45 cases of patients received standard quadruple anti-Hp treatment for 6 months were enrolled in control group,34 cases of patients those received probiotics combined with standard quadruple anti-Hp treatment were enrolled in the study group,and the course of the treatment was 6 months too.The clinical symptoms,Hp removal rate,and side reactions of the two groups were statistically analyzed.Results The data of clinical efficacy met"ball symmetry"hypothesis,and within group variance analysis was conducted and the results indicated that:there was statistically significant difference in the efficacy between the two groups(P<0.05).The incidence of side effects in the study group was lower than that in the control group(11.76%&35.56%),and the difference was statistically significant(P<0.05).The follow up was conducted 4 weeks after the treatment,unfortunately 1 case was lost to follow-up in the control group,Hp clearance rate of the study group was higher than the control group(94.12%&77.27%),with a statistically significant difference(P<0.05).Conclusions The application of the scheme combination of probiotic and quadruple anti-Hp drugs increases the removal rate of Hp,improves the clinical symptoms of elderly patients with gastric ulcers,significantly declines the drug-related side effects,it has important clinical practical application value,is worth promoting and applying.
