针吸细胞学对乳腺黏液癌的诊断价值

摘　　要：目的探讨乳腺黏液癌的针吸细胞形态学特点和鉴别诊断。方法结合临床及组织病理学资料,回顾性分析2017—2019年本院收治的针吸细胞学诊断的5例乳腺黏液癌患者的细胞学资料,结合相关文献总结乳腺黏液癌的细胞形态学诊断特征。结果以病理学结果为金标准,5例乳腺黏液癌针吸细胞学诊断结果全部与组织病理学结果一致,准确率为100%。5例乳腺黏液癌针吸物均为半透明胶冻样黏稠物,镜下形态学特点:癌细胞体积轻微增大、轻度异型,呈乳头状或三维立体球团样紧密排列,背景可见大量粘液样物质,可形成黏液湖,癌细胞团如同小岛漂浮在黏液湖中。结论乳腺黏液癌是一种少见但具有独特针吸细胞形态学特征的肿瘤,FNAC诊断准确率高,可以作为乳腺黏液癌术前诊断的一种常规检查方法。Objective To investigate the fine needle morphological characteristics of cells of mucinous breast carcinoma and differential diagnosis with other breast tumors.Methods Combined with clinical and histopathological data,the cytological data of 5 patients with mucinous breast carcinoma diagnosed by fine-needle aspiration cytology those admitted to our hospital from year 2017 to 2019 were retrospectively analyzed.The diagnosed characteristics of cytomorphology of mucinous breast carcinoma were reviewed combined with related literatures.Results Pathological results were set as golden standard,the results of needle aspiration cytology of the 5 cases of mucinous breast carcinoma were all consistent with the histopathological results,and the accuracy was 100%.The samples of needle aspirates of the 5 cases of mucinous breast cancer were all semi-transparent gelatinous viscous substances.Microscopically,the morphological features were as follows:the tumor cells were slightly enlarged in volume and slightly deformed,presenting a close arrangement of papillary or three-dimensional pellets.In the background,a large number of mucoid substances could be seen,forming a mucous lake,and the tumor cells were like islands floating in the mucous lake.Conclusions Mucinous breast carcinoma is a rare but distinctive neoplasm that could be accurately diagnosed by fine needle aspiration cytology,which could be used as a routine method for preoperative diagnosis of mucinous breast carcinoma.
