摘　　要：目的探讨阑尾黏液性囊腺瘤的MDCT表现特征并与病理进行分析,提高对该病的诊断准确性。方法选取经病理证实的21例阑尾黏液性囊腺瘤的CT轴位及多平面重组(MPR)后处理的图像特征,与手术病理结果进行对照分析。结果21例中,CT轴位表现为类圆形或椭圆形13例,长管状3例,空腔形成2例,分叶状1例,不规则形2例;MPR表现为囊袋状8例,葫芦状5例,腊肠状4例,梭形3例,不规则形1例。12例囊壁厚薄均匀,9例囊壁厚薄不均,内壁见小结节影,9例囊壁见钙化,囊内容物密度均匀或不均匀,5例囊内见分隔。12例动态增强扫描表现为囊壁、小壁结节及囊内分隔呈轻中度持续性强化。9例病灶周边感染及渗出,3例病灶合并淋巴结肿大。结论阑尾黏液性囊腺瘤的MDCT表现具有一定的特征性,结合患者的临床资料可提高本病的检出率和诊断符合率。Objective To study the MDCT features of mucinous cystadenoma of appendix and compare with the pathological results,so as to improve the recognition and to diagnosis accuracy of the disease.Methods The image features of CT axial and MPR postprocessing in 21 cases of appendiceal mucinous cystadenoma confirmed by pathology were analyzed retrospectively,and compared with the results of operation and pathology.Results In 21 cases,CT axial features were round or oval in 13 cases,long tube in 3 cases,cavity in 2 cases,lobulated in 1 case,irregular in 2 cases;MPR showed that lesions were pouch in 8 cases,gourd in 5 cases,sausage in 4 cases,fusiform in 3 cases,and irregular in 1 case.In 12 cases,the thickness of the cyst wall was uniform,in 9 cases,the thickness of the cyst wall was uneven,small nodular shadow was seen on the inner wall.In 9 cases,calcification was seen on the wall,the density of the contents of the cyst was uniform or uneven,and in 5 cases,the separation was seen in the cyst.In 12 cases,the dynamic contrast-enhanced scan showed that the cystic wall,small wall nodule and the internal septum were enhanced in a light to medium degree.There were 9 cases of peripheral infection and exudation and 3 cases of lymphadenopathy.Conclusion MDCT findings of mucinous cystadenoma of appendix have certain characteristics.Combined with the clinical data of patients,the detection rate and diagnostic coincidence rate of this disease can be improved.
