术前检测前列腺癌患者血清miR-148a-3p及miR-139-5p表达水平预测术后复发转移的价值

摘　　要：目的探讨术前检测前列腺癌患者血清miR-148a-3p及miR-139-5p表达水平对预测术后复发转移的价值。方法选择2016年10月至2019年10月平煤神马医疗集团总医院收治的120例前列腺癌患者作为研究对象。按照在随访期内是否出现复发转移分为复发转移组(47例)与术后未复发转移组(73例)。比较两组患者血清miR-148a-3p及miR-139-5p表达水平;分析术后复发转移患者血清miR-148a-3p及miR-139-5p的表达水平与临床病理学特征之间的关系;分析血清miR-148a-3p及miR-139-5p水平在预测前列腺癌术后复发转移的受试者工作特征曲线图。结果术后复发转移组血清miR-148a-3p及miR-139-5p相对表达水平均高于术后未复发转移组(P均<0.05)。术后复发转移患者血清miR-148a-3p及miR-139-5p表达水平与肿瘤TNM分期、Gleason评分均具有显著的相关性(P均<0.05)。miR-148a-3p及miR-139-5p两项指标绘制受试者工作特征曲线分析结果表明,两项指标曲线下面积差异均有统计学意义(P均<0.01)。miR-148a-3p表达水平取4.45 ng/ml时的敏感度为91.5%,特异度为83.2%;miR-139-5p表达水平取7.34 ng/ml时的敏感度为73.5%,特异度为79.2%。结论术后复发转移前列腺癌患者术前血清miR-148a-3p及miR-139-5p相对表达水平显著上升,且与TNM分期、Gleason评分呈正相关,二者在预测前列腺癌术后复发转移中的敏感度及特异度均较好,对临床评估前列腺癌患者预后状况具有一定的价值。Objective To investigate the value of preoperative detection of serum miR-148a-3p and miR-139-5p expression levels in predicting postoperative recurrence and metastasis in patients with prostate cancer.Methods A total of 120 prostate cancer patients admitted to General Hospital of Pingmei Shenma Medical Group from October 2016 to October 2019 were selected as the research objects.According to the recurrence and metastasis in the follow-up period,the patients were divided into recurrence and metastasis group(47 cases),and non recurrence and metastasis group(73 cases).The serum miR-148a-3p and miR-139-5p expression levels were compared between the two groups.The correlations of the serum miR-148a-3p and miR-139-5p expression levels with clinic opathological characteristics in patients with postoperative recurrence and metastasis were analyzed.The receiver operating characteristic(ROC)curve of miR-148a-3p and miR-139-5p levels in predicting recurrence and metastasis after prostate cancer surgery was analyzed.Results The relative expression levels of serum miR-148a-3p and miR-139-5p in recurrence and metastasis group were higher than those in non recurrence and metastasis group(all P<0.05).The expression levels of serum miR-148a-3p and miR-139-5p in 47 patients with recurrence and metastasis postoperative were significantly correlated with tumor,necrosis and metastasis(TNM)staging,and Gleason score(all P<0.05).The results of ROC curves of miR-148a-3p and miR-139-5p showed that the difference of the area under the curve of the two indicators was statistically significant(all P<0.01).When the expression level of miR-148a-3p was 4.45 ng/ml,the sensitivity was 91.5%,and the specificity was 83.2%.The sensitivity and specificity of miR-139-5p at 7.34 ng/ml were 73.5%and 79.2%respectively.Conclusions The preoperative relative expression levels of serum miR-148a-3p and miR-139-5p in patients with postoperative recurrence and metastasis of prostate cancer increase significantly,and they are positively correlated with TNM stag
