DRGs在肾脏及泌尿系统疾病住院医疗绩效评价中的应用

摘　　要：目的应用诊断相关组对医院学科发展不足领域肾脏及泌尿系统疾病进行住院医疗绩效评价,为医院加强精细化管理和学科建设提供数据支持。方法以国家版DRGs(CN-DRGs)分组方式,运用CMI、DRGs组数、费用消耗指数、时间消耗指数、低风险组死亡率等指标对2015年-2019年医院出院病例进行分析。结果2015年-2019年医院肾脏及泌尿系统疾病及功能障碍综合能力提高,DRGs组数逐渐增多,2019年达到最高40组;费用、时间消耗指数降低,费用消耗指数2019年达到最低0.69,时间消耗指数2019年达到最低0.89;CMI降低,2019年最低0.85;低风险死亡率升高,2019年出现一例低风险死亡病例。结论医院近五年,泌尿外科及肾内科能收治大多数患者,医疗服务范围广,医疗服务效率高,但同时应提高疑难病种收治,注意医疗服务的安全性。Objective By using the diagnosis related groups,to evaluate clinical service performance of Nephrology and urology,and to provide data support for fine management and discipline construction.Methods According to CN-DRGs packet mode,case-mix index(CMI),DRGs number,cost consumption index,time consumption index,and mortality rate of low-risk group were used to analyze the discharged cases of research hospital from 2015 to 2019.Results From 2015 to 2019,research hospital MDCL comprehensive capacity improved,the overall DRGs number gradually increased,it reached the highest 40 groups in 2019;the cost-consuming index and the time-consuming index decreased,the cost-consuming index reached the lowest 0.69 in 2019,and the time-consuming index reached the lowest 0.89 in 2019;CMI decreased to 0.85 in 2019;and the low risk of mortality increased,there was a low risk death case in 2019.Conclusion Research hospital for nearly five years,Urology and nephrology can treat most patients,the wide range of medical services should maintain the advantage,and the efficiency of medical services are high.But at the same time,we should improve the treatment of difficult diseases,and pay attention to the safety of medical services.
