MRI对于早期肝性脑病患者的诊断价值及准确性分析

摘　　要：目的:分析磁共振(MRI)诊断早期肝性脑病的临床应用价值和准确性。方法:2018年8月-2020年4月收治疑似早期肝性脑病患者80例,均接受MRI和多层螺旋CT检查;将活检结果及手术病理检查结果作为金标准,比较两者联合检查与MRI检查结果。结果:联合检查准确率略高于MRI检查,但差异无统计学意义(P>0.05);联合检查肝硬化分级准确率高于MRI检查,差异有统计学意义(P<0.05)。对16例急性肝性脑病患者进行检查时发现,存在中脑腹侧高信号以及双侧基底节苍白球区高信号现象11例,出现弥漫性脑水肿改变现象4例;对60例慢性肝性脑病患者进行检查时发现,存在双侧基底节苍白球区高信号现象40例,存在中脑腹侧高信号现象10例;存在垂体前叶高信号现象10例,还有25例患者在检查时发现存在脑萎缩症状。结论:对早期肝性脑病通过MRI诊断的应用价值很高,在准确性上也有着很好的保证。Objective:To explore the diagnostic value and accuracy of MRI in patients with early hepatic encephalopathy.Methods:From August 2018 to April 2020,80 patients with suspected early hepatic encephalopathy were selected,all of them underwent MRI and multi-slice spiral CT examination.The results of biopsy and surgical pathology were taken as the gold standard,and the results of combined examination and MRI were compared.Results:The accuracy of combined examination was slightly higher than that of MRI,but the difference was not statistically significant(P>0.05).The accuracy of liver cirrhosis grading of combined examination was higher than that of MRI,the difference was statistically significant(P<0.05).16 patients with acute hepatic encephalopathy were examined,there were 11 cases of high signal in ventral midbrain and bilateral basal ganglia pallidum,and 4 cases of diffuse brain edema.During the examination of 60 patients with chronic hepatic encephalopathy,we found that there were 40 cases of high signal in bilateral basal ganglia pallidum and 10 cases of high signal in ventral midbrain.There were 10 cases of anterior pituitary hyperintensity,and 25 cases of brain atrophy were found in the examination.Conclusion:It has high application value in the diagnosis of early hepatic encephalopathy by MRI,and also has a good guarantee in accuracy.
