摘 要:目的:分析^(18)F-FDG符合线路显像在肺占位性病变良、恶性辨别中的临床诊断价值。方法:选择2016年2月-2018年12月93例肺占位性病变患者,所有患者经其他胸部影像学检查发现,回顾总结分析其^(18)F-FDG符合线路显像的靶/非靶比值(T/NT)的差异,利用SPSS 19.0统计软件行二元logistic回归分析,并绘制ROC曲线,计算求出约登指数及对应最佳诊断截值。结果:共选取93例患者,最终病理结果显示22例属良性病变,71例属恶性肿瘤。利用SPSS 19.0进行二元logistic分析显示,T/NT会显著影响病理结果的良恶性;绘制ROC曲线,AUC=0.853,约登指数=0.586,对应T/NT截断值(cut-off值)为2.94,则计算出T/NT≥2.94者67例,T/NT<2.94者26例;计算该方法诊断肺占位性病变的诊断效能:灵敏度(85.92%)、特异度(72.73%)、阳性预测值(91.04%)、阴性预测值(61.54%)及准确性(82.80%),此方法与病理诊断结果比较无统计学差异(P=0.503)。结论:^(18)F-FDG符合线路显像方法在辨别诊断肺占位性病变的良恶性时,是较为重要的诊断方法之一。Objective:To retrospectively analyze the clinical diagnostic value of ^(18)F-FDG coincidence imaging in distinguishing benign and malignant lung occupying lesions.Methods:A total of 93 patients with lung mass lesions were selected from February 2016 to December 2018.All patients were found by other chest imaging examinations,and the target/non-target ratio(T/NT)of ^(18)F-FDG in line with line imaging was retrospectively analyzed.SPSS 19.0 statistical software was used to perform binary logistic regression analysis,and the ROC curve was drawn to calculate the Jordan index and the corresponding best diagnostic cut-off.Results:In this summary analysis,a total of 93 patients were selected.The final pathological results showed that 22 cases were benign lesions and 71 cases were malignant tumors.Binary logistic analysis using SPSS 19.0 showed that T/NT will significantly affect the benign and malignant pathological results.ROC curve was drawn,AUC=0.853,Jordan index=0.586.Corresponding T/NT cut-off value was 2.94.67 cases with T/NT≥2.94 and 26 cases with T/NT<2.94 were calculated.The diagnostic efficacy of this method in the diagnosis of lung mass lesions was calculated:Sensitivity(85.92%),specificity(72.73%),positive predictive value(91.04%),negative predictive value(61.54%),and accuracy(82.80%).There was no statistical difference between this method and the pathological diagnosis results(P=0.503).Conclusion:^(18)F-FDG coincidence imaging is one of the more important diagnostic methods in the diagnosis of benign and malignant lung occupying lesions.
