摘 要:目的:分析总结慢性淋巴细胞白血病(CLL)/小淋巴细胞淋巴瘤(SLL)的氟代脱氧葡萄糖正电子发射计算机断层显像(fluorine-18 flurodeoxyglucose positron emission tomography/computedtomography,^(18)F-FDG PET/CT)特征,以提高对本病的认识。方法:回顾性分析2017年9月-2019年5月笔者所在医院经骨穿、淋巴结活检病理及临床治疗随访证实的7例CLL/SLL患者^(18)F-FDG PET/CT显像结果,分析其CT及PET影像特征。结果:7例患者均见全身多发淋巴结肿大,呈弥漫性密集分布、密度均比较均匀,无明显坏死、融合、血管包绕征象;PET大部分淋巴结未见明显放射性异常浓聚,部分轻度放射性异常浓聚;脾脏、骨骼及骨髓代谢均未见明显增高。结论:^(18)F-FDG PET/CT表现为全身多发淋巴结肿大,淋巴结弥漫密集分布、密度均匀、代谢可无增高或轻度增高,脾脏、骨髓及结外脏器较少累及,应考虑到CLL/SLL可能。Objective:To analyze and summarize the characteristics of fluorine-18 flurodeoxyglucose positron emission tomography/computedtomography(^(18)F-FDG PET/CT)in chronic lymphocytic leukemia(CLL)/small lymphocytic lymphoma(SLL)in order to improve the understanding of this disease.Method:The^(18)F-FDG PET/CT imaging results of 7 CLL/SLL patients confirmed by bone penetration,lymph node biopsy pathology and clinical follow-up from September 2017 to May 2019 in our hospital were retrospectively analyzed,and the CT and PET imaging characteristics were analyzed.Result:All the 7 patients showed multiple systemic lymph node enlargement,with diffuse dense distribution and relatively uniform density,without obvious signs of necrosis,fusion or vascular envelopment.Most of the lymph nodes of PET showed no obvious abnormal radioactivity concentration,and some of them had mild abnormal radioactivity concentration.There was no significant increase in spleen,bone and bone marrow metabolism.Conclusion:^(18)F-FDG PET/CT shows multiple systemic lymph node enlargement,dense distribution and uniform density of lymph nodes,no increase or slight increase in metabolism,and less involvement of spleen,bone marrow and extracellular organs,CLL/SLL should be considered.
