后腹腔镜与开放肾部分切除术对局限性肾肿瘤的治疗效果和安全性对比

摘　　要：目的:分析后腹腔镜肾部分切除术(RLPN)、开放性肾部分切除术(OPN)在局限性肾肿瘤中的应用价值。方法:选取本院2018年9月-2019年11月收治的107例局限性肾肿瘤患者,以随机分组法分为开放组(53例)和腔镜组(54例),分别行OPN治疗、RLPN治疗。比较两组疗效、手术情况、康复情况、并发症发生情况、生活质量。结果:两组治疗总有效率对比差异无统计学意义(P>0.05);腔镜组术中出血量低于开放组(P<0.05);腔镜组手术时长及术中肾热缺血时间均长于开放组(P<0.05);腔镜组肾窝总引流量、胃肠功能恢复时间及术后住院时间均优于开放组(P<0.05);腔镜组并发症发生率低于开放组(P<0.05)。腔镜组手术后1周生活质量评分高于开放组(P<0.05)。结论:局限性肾肿瘤治疗中,RLPN与OPN均可获得良好的疗效,但RLPN术式的创伤较小,操作更加精细,安全性更高。Objective:To analyse of retroperitoneal laparoscopic partial nephrectomy(RLPN)and open partial nephrectomy(OPN)in localized renal tumors.Method:A total of 107 patients with localized renal tumors admitted to our hospital from September 2018 to November 2019 were selected,they were randomly divided into the open group(53 cases)and the endoscopy group(54 cases),and received OPN treatment and RLPN treatment respectively.The efficacy,operation,rehabilitation,complications and quality of life were compared between the two groups.Result:There was no significant difference in the total effective rate between the two groups(P>0.05).The intraoperative blood volume in the endoscopy group was lower than that in the open group(P<0.05).The operation time and the intraoperative renal warm ischemia time in the endoscopy group were longer than those in the open group(P<0.05).The total drainage volume of the renal fossa,the recovery time of gastrointestinal function and the postoperative hospitalization time in the endoscopy group were better than those in the open group(P<0.05).The incidence of complications in the endoscopy group was lower than that in the open group(P<0.05).The quality of life score at 1 week after surgery in the endoscopy group was higher than that in the open group(P<0.05).Conclusion:Both RLPN and OPN can achieve good results in the treatment of localized renal tumors,but the operation of RLPN is less invasive,more precise and safer.
