PEG-rhG-CSF与G-CSF预防及治疗化疗后中性粒细胞缺乏的效果比较

摘　　要：目的:回顾性分析PEG-rhG-CSF与G-CSF在淋巴瘤化疗后中性粒细胞缺乏预防中的作用,及其不良反应对比。方法:回顾性选取诊断明确的淋巴瘤患者共78例,其中在淋巴瘤化疗后立即给予PEG-rhG-CSF预防中性粒细胞缺乏的患者42例(实验组),另外36例患者则是在出现中性粒细胞缺乏后给予G-CSF治疗(对照组),直至粒细胞>2.0×10^(9)/L。对比PEG-rhG-CSF与G-CSF在化疗后粒细胞缺乏中的预防及治疗作用,观察中性粒细胞平均最低值,中性粒细胞缺乏的持续时间,以及4级FN的发生概率,并观察两组发热、肺内感染、感染相关指标以及不良反应的发生概率。结果:两组患者的化疗前KPS评分无统计学差异,中性粒细胞平均最低值在对照组明显低于实验组,中性粒细胞缺乏的持续时间在实验组明显缩短,4级FN的发生率在实验组明显降低,并且不良反应在两组无明显差异。结论:PEG-rhG-CSF与G-CSF比较,在不增加不良反应的同时,缩短了中性粒细胞缺乏的持续时间及4级FN的发生概率,降低了感染的概率,减轻了患者的痛苦,增加了患者的依从性。Objective:To retrospectively analyze the role of PEG-rhG-CSF and G-CSF in the prevention of neutrophil deficiency after chemotherapy in lymphoma and compare the adverse reactions of PEG-rhG-CSF and G-CSF.Methods:78 patients diagnosed with lymphoma were selected.42 patients were given PEG-rhG-CSF to prevent neutrophil deficiency immediately after chemotherapy,and 36 patients were given G-CSF to prevent neutrophil deficiency until neutrophil>2.0×10^(9)/L.The preventive and therapeutic effects of PEG-rhG-CSF and G-CSF on neutrophil deficiency after chemotherapy were compared,and the average minimum value of neutrophils was observed.The duration of neutrophil deficiency and the probability of FN(grade 4)were observed.The incidence of fever,pulmonary infection and drug adverse reactions were also observed.Results:There was no statistical difference in KPS score between the two groups before chemotherapy.The average minimum value of neutrophils in the control group was significantly lower than that in the experimental group.The duration of neutrophil deficiency was significantly shortened in the experimental group.The incidence of FN(grade 4)was significantly reduced in the experimental group,and there was no significant difference in adverse reactions between the two groups.Conclusion:Compared with G-CSF,PEG-rhG-CSF shortens the duration of neutrophil deficiency,reduces the probability of infection,alleviates the pain of patients and increases their compliance without increasing adverse reactions.
