胃镜下组织胶注射联合止血夹治疗食管胃底曲张静脉破裂出血的临床研究

摘　　要：目的:探讨胃镜下组织胶注射联合止血夹治疗食管胃底曲张静脉破裂出血的临床效果。方法:选取2019年2月-2020年1月笔者所在医院收治的食管胃底曲张静脉破裂出血患者84例作为研究对象,随机分成观察组(n=42)和对照组(n=42),对照组采用药物保守治疗,观察组采用胃镜下组织胶注射联合止血夹进行治疗。比较两组止血有效率与止血时间、6个月后再出血率及食管静脉曲张程度、不良反应发生率。结果:观察组止血有效率高于对照组,止血时间短于对照组,差异均有统计学意义(P<0.05);观察组食管静脉曲张缓解率高于对照组,再出血率低于对照组,差异均有统计学意义(P<0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:食管胃底曲张静脉破裂出血采用胃镜下组织胶注射联合止血夹治疗,可提高止血有效率,减少止血时间,提高止血效率,降低再出血率和不良反应发生率,临床疗效显著,值得推广使用。Objective:To investigate the clinical effect of endoscopic tissue glue injection combined with hemostatic clip in treatment of esophageal and gastric variceal vein rupture and hemorrhage.Method:A total of 84 patients with rupture and bleeding of esophagogastric varices admitted to our hospital from February 2019 to January 2020 were selected as the research object,and randomly divided into the observation group(n=42)and the control group(n=42).The control group was treated with conservative drugs,and the observation group was treated with endoscopic tissue glue injection combined with hemostatic clip.The effective rate of hemostasis,hemostasis time,rebleeding rate after 6 months,degree of esophageal varicose veins and incidence of adverse reactions were compared between the two groups.Result:The effective rate of hemostasis in the observation group was higher than that in the control group,and the time of hemostasis in the observation group was shorter than that in the control group,the differences were statistically significant(P<0.05).The remission rate of esophageal varices in the observation group was higher than that in the control group,and the rebleeding rate was lower than that in the control group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:The treatment of esophageal and gastric variceal vein rupture and hemorrhage by gastroscopy with tissue glue injection combined with hemostatic clips can increase the effective rate of hemostasis,reduce the time of hemostasis,increase the efficiency of hemostasis,reduce the rate of rebleeding and the incidence of adverse reactions,the curative effect is remarkable,and it is worth popularizing.
