摘 要:目的:探讨环形超声支气管镜(R-EBUS)与虚拟导航(VBN)联合快速现场检测技术(ROSE)能否减少肺外周病变诊断期间的医疗资源消耗。方法:将2018年11月—2019年3月收治的100例肺外周型病变患者在应用VBN联合R-EBUS的基础上随机分成联合观察组(ROSE组)及对照组(非ROSE组)。统计分析两组患者诊断阳性率、首次活组织检查(以下简称活检)至确诊费用、首次活检至确诊时间。结果:观察组50例,诊断阳性率84.00%(42/50),首次活检至确诊中位诊疗费用为1425.75元,首次活检至确诊时间为(3.84±4.28)d。对照组50例,诊断阳性率74.00%(37/50),首次活检至确诊中位诊疗费用为5162.00元,首次活检至确诊时间时间(6.46±3.66)d。结论:R-EBUS与VBN联合ROSE可以减少诊断肺外周病变的医疗资源消耗。Objective:To analyze the difference betweenmedical resource consumption of radial endobronchial ultrasound(R-EBUS)and virtual bronchoscopic navigation(VBN)combined with or without rapid on-site evaluation(ROSE)in the diagnosis of peripheral pulmonary disease.Methods:All 100 patients with peripheral pulmonary disease admitted in our department from November 2018 to March 2019 were randomly divided into two groups to accept VBN and R-EBUS combined with ROSE biopsy(experimental group)and without ROSE biopsy(control group).The difference in the cost from the first biopsy to the diagnosis,and the time from the first biopsy to the diagnosis in both groups were analyzed with statistical software.Results:There were 50 patients in the experimental group.The diagnostic positive rate was 84.00%(42/50).The median cost from the first biopsy to the confirmed diagnosis was 1425.75 yuan.The average time from the first biopsy to the confirmed diagnosis was(3.84±4.28)days.There were 50 patients in the control group.The positive diagnosis rate was 74.00%(37/50).The median cost from the first biopsy to the confirmed diagnosis was 5162.00 yuan.The average time from the first biopsy to the confirmed diagnosis was(6.46±3.66)days.Conclusion:The combination of R-EBUS,VBN and ROSE can save the medical resources required for the diagnosis of peripheral pulmonary disease.
