摘 要:目的探讨声触诊组织成像定量技术(virtual touch tissue imaging quantification,VTIQ)联合超声造影(contrast-enhanced ultrasound,CEUS)对乳腺影像报告和数据系统(Breast Imaging Reporting and Data System,BI-RADS)4类乳腺肿瘤的鉴别诊断价值。方法选取常规超声诊断为BI-RADS 4类的乳腺肿瘤患者80例,以术后病理结果为金标准,比较分析VTIQ、CEUS及二者联合应用对BI-RADS 4类乳腺肿瘤的鉴别诊断效能。结果纳入研究的BI-RADS 4类乳腺肿瘤80例中,良性肿瘤48例,恶性肿瘤32例。VTIQ技术鉴别诊断BI-RADS 4类乳腺良恶性肿瘤的敏感度为71.91%,特异度为75.02%,准确度为71.25%。CEUS鉴别诊断的敏感度为75.00%,特异度为81.25%,准确度为72.50%。VTIQ联合CEUS鉴别诊断的敏感度为92.59%,特异度为82.22%,准确度为92.50%。VTIQ联合CEUS对BI-RADS 4类乳腺肿瘤的诊断效能高于单独使用VTIQ或CEUS(均P<0.05)。结论 VTIQ联合CEUS技术有助于提高BI-RADS 4类乳腺肿瘤的诊断准确度。Objective To explore the differential diagnostic value of virtual touch tissue imaging quantification(VTIQ)combined with contrast-enhanced ultrasound(CEUS)for Breast Imaging Reporting and Data System(BI-RADS)4 category breast lesions.Methods Eighty patients with BI-RADS 4 category breast lesions were recruited.With surgical pathology diagnosis as the gold standard,the diagnostic efficacy of VTIQ,CEUS and VTIQ combined with CEUS in BI-RADS 4 category breast lesions were compared and analyzed.Results Among the 80 cases of BI-RADS category 4 breast lesions included in this study,48 were benign lesions and 32 were malignant lesions.The sensitivity,specificity and accuracy of VTIQ were 71.91%,75.02%and 71.25%in differential diagnosis of BI-RADS category 4 benign and malignant breast lesions.The sensitivity,specificity and accuracy of CEUS for differential diagnosis were 75.00%,81.25%and 72.50%.The sensitivity,specificity and accuracy of VTIQ combined with CEUS for differential diagnosis were 92.59%,82.22%and 92.50%.The diagnostic efficacy of VTIQ combined with CEUS in the diagnosis of BI-RADS category 4 breast lesions was significantly higher than that of VITQ or CEUS alone(both P<0.05).Conclusion VTIQ combined with CEUS is helpful to improve the diagnostic accuracy of BI-RADS 4 category breast lesions.
