睾丸胚胎癌的影像学表现及鉴别诊断

摘　　要：目的探讨睾丸胚胎癌的影像学表现,提高对该病的认识。方法回顾性分析9例经术后病理证实为睾丸胚胎癌患者的临床及影像学资料。其中3例行CT平扫,5例行CT平扫+增强,6例行MRI平扫+增强。结果6例患者甲胎蛋白(AFP)增高,9例患者β-人绒毛膜促性腺激素(β-HCG)均升高。CT及MRI所有病灶均表现为混杂信号或密度影,增强为不均匀强化。1例病灶内可见条片状钙化影;7例有远处转移,其中1例发生肺转移,6例为腹膜后或腹股沟区淋巴结转移;5例病变累及精索。结论睾丸胚胎癌影像学多表现为病灶侧精索受累并增粗,腹膜后淋巴结肿大并融合成片。Objective To investigate the imaging features of testicular embryonal carcinoma and improve the understanding of the disease.Methods The clinical and imaging data of 9 patients with testicular embryonal carcinoma confirmed by postoperative pathol-ogy were analyzed retrospectively.Three of them underwent plain CT scans,5 cases received plain and enhanced CT scans,and 6 cases was performed plain and enhanced scans of MRI.Results Alfa-fetoprotein.(AFP)was increased in 6 cases,Beta-human chorionic gonadotropin(β-HCG)were increased in 9 cases.AII lesions on CT and MRI showed mixed inhomogeneous signals or uneven density shadow,which all of these lesions were heterogeneous enhancement.Stripe calcification was seen in 1 case,7 cases had distant metas-tasis,1 of which had lung metastasis,6 cases had retroperitoneal or groin lymph nodes metastasis.The spermatic cord was involved in 5 cases.Conclusion The imaging findings of testicular embryonal carcinoma usually show that spermatic cord is involved and thick-ened on the lesion side。and the retroperitoneal lymph nodes are enlarged and fused into slices.
