应用CBCT研究不同矫治器正畸拔牙矫治对前牙区牙根及牙槽骨的影响

摘　　要：目的应用CBCT研究无托槽隐形矫治及传统固定矫治器正畸拔牙对前牙区牙根及牙槽骨的变化情况。方法选择成人拔牙矫正的患者60例,随机分为无托槽隐形矫治器组及传统固定矫治器组(N=30)。正畸治疗前后拍摄CBCT,测量两组正畸患者上下颌中切牙、侧切牙、尖牙的牙根长度及牙槽骨的厚度变化情况。结果固定矫治器组较无托槽隐形矫治器组上下中切牙、侧切牙牙根吸收变明显,差异有统计学意义,尤其上颌侧切牙牙根吸收更明显,但在两组中上下尖牙牙根长度变化无统计学意义。两组矫治器牙槽骨厚度变化无统计学意义。结论正畸拔牙病例中,无托槽隐形矫治器较固定矫治器组对牙根吸收的影响小,牙槽骨改建基本一致。Objective To investigate the effects of bracket-free invisible appliance and traditional fixed appliances on tooth roots and alveolar bone in the anterior region in orthodontic extraction cases with cone-beam CT(CBCT).Methods A total of 60 adult patients were selected for orthodontic treatment after tooth extraction,and they were randomly divided into two groups:bracket-free invisible appliance group(N=30)and traditional fixed appliance group(N=30).The CBCT scans of the patients were obtained before and after orthodontic treatment to measure the thickness of alveolar bone and root lengths of central incisors,lateral incisors,and canines.Results The root resorption of the central incisors and lateral incisors in the fixed appliance group was more obvious than that in the invisible appliance group without brackets.In the meantime,the difference in root resorption of maxillary lateral incisor in the two groups was even more remarkable.By contrast,there was no significant difference in the changes in the root lengths of maxillary and mandible canines of the two groups and the changes in the thickness of the alveolar bone of the two groups.Conclusion In orthodontic extraction cases,the effect of the bracket-free invisible appliance on root resorption is significantly less than that of the fixed appliance,while the two appliances have a similar influence on alveolar bone reconstruction.
