摘 要:目的与CT血管成像(CTA)比较,探讨超声对颈动脉体瘤(CBT)的定量分型评估价值。资料与方法回顾性分析经手术病理证实的36例CBT共38个瘤体的影像学资料,所有患者均行CTA检查,其中20例22个瘤体术前行超声检查;采用Pearson法分析二维超声与CTA对患者术前瘤体最大径测值与术后病理测值的相关性,比较两者对CBT分型与术中分型的一致性。结果20例18个组织完整的瘤体术前二维超声最大径测值为1.9~7.8 cm,中位数3.5(2.6,4.7)cm;CTA测值为2.0~7.2 cm,中位数3.5(2.6,4.3)cm;术后病理测值为2.0~8.0 cm,中位数3.4(2.6,5.0)cm。二维超声及CTA对CBT的测值均与术后病理测值相关(r=0.86、0.75,P均<0.01)。术前超声、CTA分别将1例和2例ShamblinⅡ型误判为Ⅲ型。结论超声在评估CBT大小和分型方面有较好的应用价值。Purpose To evaluate the value of ultrasound in the quantification and classification of carotid body tumor(CBT)compared with computed tomography angiography(CTA).Materials and Methods The imaging data of 38 tumors in 36 patients with CBT confirmed by pathology were analyzed retrospectively.All patients were examined by CTA,of which 22 tumors in 20 patients were examined by ultrasound before operation.Pearson analysis was used to analyze the correlation between preoperative maximum diameter measured by two-dimensional ultrasound and CTA and postoperative pathological maximum diameter,the consistency in CBT classification between preoperative and intraoperative evaluation was compared.Results The maximum diameter of 18 tumors in 20 patients with intact tissues measured by two-dimensional ultrasound before operation was 1.9-7.8 cm,the median was 3.5(2.6,4.7)cm,while that measured by CTA was 2.0-7.2 cm,the median was 3.5(2.6,4.3)cm;the postoperative pathological value was 2.0-8.0 cm,the median was 3.4(2.6,5.0)cm.The values of CBT measured by two-dimensional ultrasound and CTA were correlated with postoperative pathological measurements(r=0.86 and 0.75,respectively,all P<0.01).One case of ShamblinⅡ(intraoperative classification)was misdiagnosed as ShamblinⅢby ultrasound,while two cases of ShamblinⅡwas misdiagnosed as ShamblinⅢby CTA.Conclusion Ultrasound has high application value in evaluating the size and classification of CBT.
