甲状腺乳头状癌中PDK1、ZEB1和Vimentin的表达及其临床意义

摘　　要：目的:探讨磷酸肌醇依赖性蛋白激酶1(PDK1)、E盒结合锌指蛋白1(ZEB1)和波形蛋白(Vimentin)在甲状腺乳头状癌(PTC)组织中的表达情况,并探究三指标与PTC临床病理参数的相关性。方法:免疫组化EnVision法检测206例PTC及45例癌旁组织中PDK1、ZEB1和Vimentin表达,分析其与PTC临床病理的相关性。结果:在PTC中PDK1、ZEB1及Vimentin阳性表达率显著高于癌旁甲状腺组织(P<0.05)。在腺体外侵犯的、分期较晚的、伴有淋巴结转移的PTC中PDK1、ZEB1和Vimentin阳性表达率明显高于腺体外未侵犯、分期较早和无淋巴结转移的患者。PTC中的PDK1与ZEB1呈正相关(r=0.476,P=0.031),PTC中PDK1和Vimentin1呈正相关(r=0.496,P=0.027),ZEB1和Vimentin1在PTC中的表达正相关(r=0.593,P=0.017)。结论:PDK1、ZEB1及Vimentin异常表达与PTC侵袭及转移密切有关,PDK1、ZEB1和Vimentin联合评估可能对PTC的预后判断具有重要作用。Objective:In order to explore the expressions of PDK1,ZEB1 and Vimentin protein in papillary thyroid carcinoma and their clinical significances.Methods:In this study,we analyzed the immunoexpression(EnVision)of PDK1,ZEB1 and Vimentin in 206 papillary thyroid carcinomas and 45 cases of paracancerous normal thyroid tissues in relation with clinical characteristics.Results:The immunoexpression for PDK1,ZEB1 and Vimentin protein in papillary thyroid carcinoma was significantly higher than normal thyroid tissues(P<0.05).The differences of the expression of PDK1,ZEB1 and Vimentin protein in papillary thyroid carcinomas of clinical TNM stage,extrathyroidal invasion and lymph node metastasis were significant(P<0.05).Positive correlation exists in PDK1 and ZEB1(r=0.476,P=0.031).There was positive correlation between PDK1 and Vimentin(r=0.496,P=0.027).The similar positive correlation was between ZEB1 and Vimentin(r=0.593,P=0.017).Conclusion:The abnormal expressions of PDK1,ZEB1 and Vimentin gene may promote tumor invasion and metastasis of papillary thyroid carcinoma.PDK1,ZEB1 may have an important role in diagnosis and theraphy of thyroid papillary carcinoma.Co-evaluation of PDK1,ZEB1 and vimentin might be a valuable tool for predicting thyroid papillary carcinoma patient outcome.
