摘　　要：目的运用FOCUS-PDCA质量管理工具分析病案不能及时归档的原因并采取改进措施提升病案归档率,为三级公立医院绩效考核和DRGs按病种付费做好保障。方法某医院2019年6月开始运用FOCUS-PDCA质量管理工具提升病案归档率,收集某院2019年1月1日-2019年12月31日的病案7日归档数据,比较分析实施质量管理工具前后7日归档率的变化情况。结果 FOCUS-PDCA质量管理工具应用后,某医院2019年9月1日-12月31日的病案7日归档率分别为88.48%、83.26%、90.96%和95.38%,超预期指标5.38个百分点,质量管理工具应用后病案7日归档率高于质量管理工具应用前(χ^(2)=753.69,P<0.001)。结论 FOCUS-PDCA质量管理工具是提升病案归档率,促进医院科学化、规范化管理的有效工具。Objectives Using FOCUS-PDCA quality management tool to analyze the reasons why the medical records could not be filed in time and take improvement measures to improve the filing rate of medical records, make the premise for the performance evaluation of the third level public hospitals and DRGs payment by disease. Methods A hospital began to use FOCUS-PDCA quality management tool to improve the filing rate of medical records in June 2019, collect the seven day filing data from January 1st, 2019 to December 31st, 2019, compare and analyze the change of filing rate before and after the implementation of quality management tools. Results After the implementation of the FOCUS-PDCA quality management tool, the seven day filing rates of medical records in a hospital from September 1st to December 31st, 2019 were 88.48%, 83.26%, 90.96% and 95.38%, respectively, which were 5.38 percentage points higher than expected. The seven day filing rate of medical records after the application of quality management tools was higher than that before the application of quality management tool(χ^(2)=753.69,P<0.001). Conclusions The quality management tool FOCUS-PDCA was an effective tool to improve the archiving rate of medical records and promote the scientific and standardized development of hospitals.
